1.临床资料 本文治疗的病例均具蛔虫性肠梗阻症状及阳性体征。化验室肛门采检能查到蛔虫卵而确诊为蛔虫性肠梗阻,治疗后捧出蛔虫体及症状体征消失的患者。 本文病例48例,其中男29例,女19例,年龄最1.8～13岁,平均年龄8.7岁;病程3天以上(含3天)15例,2天21例,1天12例,患儿就诊时以脐周疼痛、呕吐为主,不懂事小儿以哭闹为主,触诊脐周可扪及蛔虫团,腹壁脂肪薄者可扪及条索状蛔虫体,叩诊鼓音,听诊肠鸣音亢进,并可闻及气过水音,肛门棉签插入采检均能查到蛔虫卵,病程短者体温正常,血象嗜酸性白细胞0.2～0.5,病程长者体温达39.6℃,白细胞计数13.6×10~9/L,中性达0.86. 2.治疗方法 入院确诊后给眼清胰Ⅱ号汤煎剂:乌梅、使君子、鲜苦楝根皮、槟榔、大黄、柴胡、白芍、木香、枳壳,每日1剂,煎2次在8小时内服完,左旋米唑5mg/kg,中药用量根据患儿体质、年龄、体重、病程长短而定,因儿童饮用中药不方便,应分多次饮用,以免呕吐影响疗效,体温高的重用大黄、黄芩,并配合抗生素治疗。
