目的探讨细胞角蛋白14(CK14)基因的表达在非小细胞肺癌(NSCLC)发生、发展及转移中的意义。方法采用免疫组织化学染色法检测110例NSCLC患者癌组织、癌旁组织、正常肺组织中CK14的表达。采用Western blotting法检测12例患者新鲜组织中CK14的表达。应用非条件Logistic回归分析CK14表达水平与NSCLC患者各临床病理参数的关系。结果免疫组织化学染色结果显示,CK14在NSCLC癌组织中高表达,阳性表达率为68.18%,与癌旁组织和正常肺组织相比,差异有统计学意义(P<0.001)。CK14表达水平与NSCLC患者性别、组织类型、分化程度、分期以及淋巴结转移情况显著相关(P<0.05),与年龄无关(P>0.05)。Western blotting结果显示,CK14在NSCLC癌组织中的表达率较癌旁组织和正常肺组织高。非条件Logistic分析显示,低分化癌、病理分期Ⅱ期和Ⅲ期、CK14表达阳性是NSCLC患者发生淋巴结转移的重要危险因素(P<0.05)。结论 CK14表达在NSCLC的发生、发展和转移中发挥重要作用。CK14有望成为评估NSCLC生物学行为的重要指标。
Objective To investigate the significance of cytokeratin 14 (CK14) gene expression in the development, progression and metastasis of non-small cell lung cancer (NSCLC). Methods Immunohistochemical staining was used to detect the expression of CK14 in 110 cases of NSCLC patients with cancer tissues, adjacent non-cancerous tissues and normal lung tissues. Western blotting was used to detect the expression of CK14 in fresh tissues of 12 patients. The relationship between the expression of CK14 and the clinicopathological parameters of NSCLC patients was analyzed by using unconditional Logistic regression. Results Immunohistochemical staining showed that CK14 was highly expressed in NSCLC tissues, with a positive rate of 68.18%. The difference was statistically significant (P <0.001) compared with that of paracancer tissues and normal lung tissues. The expression of CK14 was significantly associated with gender, histological type, differentiation, stage and lymph node metastasis in NSCLC patients (P <0.05), but not with age (P> 0.05). Western blotting showed that the expression of CK14 in NSCLC was higher than that in paracancerous tissues and normal lung tissues. Non-conditional Logistic analysis showed that poorly differentiated carcinoma, stage Ⅱ and Ⅲ, CK14 positive expression of NSCLC patients with lymph node metastasis is an important risk factor (P <0.05). Conclusion CK14 expression plays an important role in the occurrence, development and metastasis of NSCLC. CK14 is expected to be an important indicator for assessing the biological behavior of NSCLC.
