【目的】研究全身运动(general movements,GMs)评估对于极低出生体重早产儿运动发育结局的预测价值。【方法】对2007年7月-2011年6月66例在康复科随访的极低体重早产儿进行全身运动评估,在纠正月龄12月龄以后根据临床诊断和Peabody运动发育量表确定其运动发育结局。计算全身运动评估的敏感度、特异度、阳性预测值和阴性预测值。【结果】全身运动评估对于异常运动发育结局(脑瘫和运动发育迟缓)的预测效度为敏感性100%、特异性98.3%,阳性预测值87.5%,阴性预测值100%。全身运动评估对于脑瘫的预测效度为敏感性100%、特异性92.1%,阳性预测值37.5%,阴性预测值100%。【结论】GMs评估对于极低出生体重早产儿的运动发育结局具有很好的预测效度,作为新型神经运动评估手段,对于极低出生体重早产儿的临床随访具有重要意义,适合在我国进一步应用和推广。
【Objective】 To investigate the predictive value of general movements (GMs) for exercise outcome in very low birth weight preterm infants. 【Method】 66 cases of very low birth weight preterm infants who were followed up in rehabilitation department from July 2007 to June 2011 were evaluated for their whole body movement. After correction of age of 12 months, their movements were determined according to clinical diagnosis and Peabody exercise scale Developmental outcome. The sensitivity, specificity, positive predictive value, and negative predictive value of the whole body motor assessment were calculated. [Results] The predictive validity of whole body exercise assessment for abnormal motor development (cerebral palsy and motor retardation) was 100% sensitivity, 98.3% specificity, 87.5% positive predictive value, and 100% negative predictive value. The predictive validity of generalized motor assessment for cerebral palsy was 100% sensitivity, 92.1% specificity, 37.5% positive predictive value, and 100% negative predictive value. 【Conclusion】 The GMs assessment has a good predictive validity for motor outcome of very low birth weight premature infants. As a new neuroimaging assessment method, it is of great significance for the clinical follow-up of preterm infants with very low birth weight and is suitable for further application in our country And promotion.
