目的探讨全自动容积超声观察前壁下段瘢痕子宫形态结构改变在预测及诊断先兆子宫破裂的价值。方法回顾分析在我院剖腹的瘢痕子宫孕妇200例,其中14例手术诊断为瘢痕子宫先兆破裂,分别绘制前壁下段子宫肌层低回声结构消失和前壁下段子宫全层厚度对瘢痕子宫先兆破裂的诊断ROC曲线。通过2χ检验子宫前壁下段肌层低回声结构消失对预测先兆子宫破裂的价值。结果子宫肌层低回声结构消失的曲线下面积为0.964,前壁下段子宫全层厚度诊断的曲线下面积为0.889,差异有显著性意义(P<0.05)。前壁下段子宫肌层结构消失在正常组和先兆子宫破裂的差异具有显著性意义(P<0.001)。结论全自动容积超声观察子宫前壁下段瘢痕结构改变对先兆子宫破裂具有较高诊断价值,值得临床推广应用。
Objective To investigate the value of automatic volume ultrasound in observing the morphological changes of scarring uterus in lower anterior wall in predicting and diagnosing threatened uterine rupture. Methods A retrospective analysis of 200 cases of cesarean scar pregnancy in our hospital, of which 14 cases were diagnosed as scar uterine rupture, were drawn lower anterior wall myometrial hypoechoic structure disappeared and the anterior wall uterine thickness of the uterus of the uterus rupture Diagnostic ROC curve. By 2χ test lower anterior uterine wall myometrial hypoechoic structure disappears to predict the value of threatened uterine rupture. Results The area under the curve of disappearance of hypoechoic structure in uterine myometrium was 0.964, and the area under the curve of uterine total thickness in the anterior segment of the anterior wall was 0.889, the difference was significant (P <0.05). The anterior wall myometrium disappeared in the normal group and the difference between the rupture of the uterus was significant (P <0.001). Conclusion Automatic volume ultrasound observation of the anterior uterine anterior segment of the lower scar changes in the structure of the uterus rupture has a high diagnostic value, worthy of clinical application.
