目的 研究动脉瘤性蛛网膜下腔出血 (SAH)病人血液真核细胞转录因子 -κB(NF -κB)与可溶性细胞间黏附分子 - 1(sICAM - 1)、内皮素 - 1(ET - 1)的动态变化规律。方法 对 30例动脉瘤性SAH病人发病后 1~ 3d、5~ 7d、9~ 11d血液NF -κB与sICAM - 1、ET - 1进行动态观察 ,用TCD检测大脑中动脉血流速度 (VMCA)。结果 动脉瘤性SAH病人血液NF-κB与sICAM - 1、ET - 1在发病后 1~ 3d、5~ 7d各均值明显高于对照组 ,术前、术后脑血管痉挛组和非脑血管痉挛组有明显差异 ,死亡组与存活组也有显著差异 ;在发病后 1~ 3dsICAM、ET - 1的变化与NF -κB的变化呈正相关。结论 动脉瘤性SAH病人血液NF -κB与sICAM - 1、ET - 1的变化与SAH病人的病情演变、脑血管痉挛程度有关 ,并可判断预后。
Objective To study the relationship between the expression of NF - κB and soluble intercellular adhesion molecule - 1 (sICAM - 1) and endothelin - 1 (ET - 1) in blood of patients with aneurysmal subarachnoid hemorrhage (SAH) The dynamics of the law. Methods The blood levels of NF - κB, sICAM - 1 and ET - 1 were measured at 1 ~ 3d, 5 ~ 7d and 9 ~ 11d after aneurysm onset in 30 patients with aneurysm. The blood flow velocity of the middle cerebral artery (VMCA) . Results The mean values of NF - κB, sICAM - 1 and ET - 1 in the blood of patients with aneurysmal SAH at 1 ~ 3d and 5 ~ 7d after onset were significantly higher than those in the control group, preoperative and postoperative cerebrovascular spasm and non - cerebral vasospasm There was a significant difference between the two groups. There was also a significant difference between death group and survival group. The changes of 1 ~ 3dsICAM and ET - 1 were positively correlated with the changes of NF - κB. Conclusion The changes of blood levels of NF-κB, sICAM - 1 and ET - 1 in patients with aneurysmal SAH are related to the evolution of the disease, the degree of cerebral vasospasm and the prognosis of patients with SAH.
