目的通过比较术前尽早使用与单纯术中使用盐酸替罗非班两种方法对老年STEMI行急诊PCI患者在疗效和安全性方面的差异,探讨急诊PCI术前及术中应用替罗非班的疗效及安全性。方法 STEMI诊断明确急诊行PCI术老年患者(≥60岁)59例,随机分为术前术中替罗非班组(实验组,n=26)和单纯术中替罗非班组(对照组,n=33)。实验组术前给予盐酸替罗非班0.15ug/(kg·min),术中在导丝通过病变后经导管冠脉内推注10ug/kg,后以0.15ug/(kg·min)维持泵入,对照组在术中给药方法相同,两组术前术后均根据患者情况应用其他药物。分析比较两组患者PCI术前后的TIMI血流分级。结果两组间的一般临床特征、病变血管数等均无统计学差异(P>0.05);两组患者的梗塞相关血管均成功再通,术后各有2例死亡病例。两组术前TIMI血流分级无差异(P>0.05);术后TIMI3级血流获得率实验组明显高于对照组(P<0.05)。结论 STEMI诊断明确急诊行PCI术治疗的老年患者,提前应用盐酸替罗非班可以在一定程度上提高心肌灌注水平。
Objective To compare the efficacy and safety of two methods of early use of preoperative and tirofiban hydrochloride alone in the treatment of elderly patients with STEMI and emergency PCI and to evaluate the efficacy and safety of tirofiban in preoperative and postoperative PCI Efficacy and safety. Methods A total of 59 elderly patients (≥60 years old) undergoing emergency PCI undergoing STEMI diagnosis were randomly divided into two groups: Tirofiban group (n = 26) and Tirofiban group (n = = 33). The experimental group was given tirofiban hydrochloride 0.15ug / (kg · min) preoperatively. During the operation, the catheter was intracoronary bolus injection of 10ug / kg after the lesion and the pump was maintained at 0.15ug / (kg · min) Into the control group in the same way of administration, two groups of patients before and after surgery are based on the application of other drugs. TIMI flow classification before and after PCI was analyzed and compared between the two groups. Results There was no significant difference between the two groups in the general clinical features and the number of vascular lesions (P> 0.05). The infarction-related blood vessels in both groups were successfully recanalized and there were 2 deaths in both groups. There was no difference in preoperative TIMI grade between the two groups (P> 0.05). The TIMI grade 3 blood flow rate in the experimental group was significantly higher than that in the control group (P <0.05). Conclusion STEMI diagnosis of emergency treatment of elderly patients with acute PCI, early application of tirofiban hydrochloride can improve myocardial perfusion to some extent.
