差异性或选择性肺通气差异性肺通气(Differential Lung Ventilation简称DLV)是指不同肺、肺叶或肺段以不同的频率、潮气量或和压力的通气方式,适用于表现为单侧性或以一侧病变为主、或某一区域病变情况下使用。此时病肺的顺应性较低,气道阻力较大,传统的通气方式治疗时大部分的潮气量被压送入健肺,造成更大的通气血流比例失调。应用PEEP可使健肺过度膨胀而病肺仍残气不足。虽然病人取侧卧位可以改善这种通气血流比例失调,但是这样一方面效果不明显,二来可能促使痰液滞留,诱发严重的肺感染,实不足取。DLV可以解决这一矛盾。这种通气方式是通过一个双腔气管内套管,放入两个不同形状及功能的送气
