目的分析新生儿败血症早期肾功能损伤的危险因素。方法纳入该院2014年1月-2015年12月收治的新生儿败血症患儿71例,其中15例出现早期肾功能损伤(21.13%),设为肾功能损伤组;其余56例为肾功能未损伤组。对比两组在不同因素下出现的比例差异,将有差异的因素进行多因素Logistic回归分析,得出新生儿败血症早期肾功能损伤的独立危险因素。结果肾功能损伤组使用呼吸机例数、有出血倾向例数、合并其他疾病例数、腋下体温、出现黄疸例数、合并肺炎或脐炎例数均高于肾功能未损伤组,尿量低于肾功能未损伤组,且差异均有统计学意义(P<0.05)。多因素Logistic回归分析显示,出血倾向、合并其他疾病、出现黄疸、尿量少、合并肺炎或脐炎为败血症患儿发生早期肾功能损伤的独立危险因素。结论败血症患儿有出血倾向、合并其他疾病、出现黄疸、尿量少、合并肺炎或脐炎可增加患儿发生早期肾功能损伤的风险。
Objective To analyze the risk factors of early renal damage in neonatal sepsis. Methods Totally 71 infants with neonatal sepsis were admitted to our hospital from January 2014 to December 2015, of which 15 cases had early renal impairment (21.13%), which was set as renal injury group. The remaining 56 cases were renal failure Injury group. Comparing the differences in the proportions of the two groups under different factors, multivariate Logistic regression analysis of the factors that had differences revealed the independent risk factors of neonatal renal damage in early sepsis. Results The number of ventilator cases, the number of patients with bleeding tendency, the number of cases with other diseases, the temperature of underarm, the number of jaundice, the number of patients with pneumonia or omphalitis were significantly higher than those without renal dysfunction and urine volume Which was lower than that of non-injured renal function group (P <0.05). Multivariate Logistic regression analysis showed that bleeding tendency, combined with other diseases, jaundice, small amount of urine, with pneumonia or omphalitis as an independent risk factor for early renal impairment in children with sepsis. Conclusion Children with sepsis have bleeding tendency, combined with other diseases, jaundice, small amount of urine, combined with pneumonia or omphalitis can increase the risk of early renal impairment in children.
