为评估联合检测血清甲胎蛋白(alpha-fetoprotein,AFP)、胸苷激酶1(thymidine kinase,TK1)和分泌型蛋白Dikkopf-1(secreted protein Dikkopf-1,DKK1)在原发性肝癌(primary hepatic carcinoma,PHC)诊断的价值。本研究以85例PHC患者、55例良性肝病患者及55例体检健康者分别作为PHC组、良性肝病组及健康对照组。分别检测上述3组人员的血清AFP、TK1、DKK1水平,进行组间比对,采用受试者工作特征(receiver operating characteristic curve,ROC)曲线及曲线下面积(the area under Roc curve,AUCROC)的方法评估各指标的诊断价值。结果显示:PHC组患者的3项指标水平最高,差异有统计学意义(p<0.05);与各项指标单独检测相比,3项指标联合检测对PHC临床诊断的灵敏度、准确性和预测概率最高(p<0.05)。综上所述,血清AFP、TK1和DKK1联合检测对临床诊断PHC具有重要价值,可作为提高临床诊断PHC灵敏度和准确性的一种重要途径。
To evaluate the efficacy and safety of combined detection of serum alpha-fetoprotein (AFP), thymidine kinase (TK1), and secreted protein Dikkopf-1 (DKK1) in primary hepatic Diagnosis of PHC. In this study, 85 cases of PHC patients, 55 cases of benign liver disease and 55 cases of healthy subjects were PHC group, benign liver disease group and healthy control group. The serum levels of AFP, TK1 and DKK1 in the three groups were detected and compared among groups. The receiver operating characteristic curve (ROC) curve and the area under Roc curve (AUCROC) Methods to assess the diagnostic value of each indicator. The results showed that the PHC group had the highest level of three indicators (p <0.05), and the sensitivity, accuracy and predictive probability of the combination of the three indicators in the clinical diagnosis of PHC were significantly higher than those of PHC alone Highest (p <0.05). In summary, the combined detection of serum AFP, TK1 and DKK1 is of great value in the clinical diagnosis of PHC, which can be used as an important way to improve the sensitivity and accuracy of clinical diagnosis of PHC.
