目的回顾性分析我院10年间初发恶性血液病的情况,以研究其发病情况及流行趋势。方法取骨髓片及血片作瑞氏染色,细胞化学染色,然后显微镜读片依据FAB标准作出诊断。结果1998年1月至2007年12月初发恶性血液病1152例。其中急性淋巴细胞白血病(ALL)269例,占23.35%;急性非淋巴细胞白血病(ANLL)409例,占35.5%;慢性粒细胞白血病(CML)161例,占13.98%;慢性淋巴细胞白血病(CLL)18例,占1.56%;浆细胞白血病(PCL)5例,占0.43%;多发性骨髓瘤(MM)91例,占7.90%;骨髓增生异常综合征(MDS)141例,占12.24%;其它58例,占5.03%。在0~9岁年龄组,ALL为首位占67.63%(94/139),在30~39岁年龄组ANLL的发病比例达到53.29%(89/167),CML发病在20岁以上明显增加,CLL的发病多在50岁以上,MM和MDS的发病在40岁以上有明显增加的趋势,在>60岁年龄组比例最高,分别为60.44%(55/91)和57.45%(81/141)。1998-2007年构成比依次为8.07%、8.25%、9.38%、8.07%、7.38%、10.68%、10.94%、10.24%、13.54%、13.45%。结论近年恶性血液病有增加的趋势。
Objective To retrospectively analyze the incidence of initial hematological malignancies in our hospital over the past 10 years to study the incidence and prevalence trends. Methods Bone marrow and blood samples were taken for Wright staining and cytochemical staining, and then the microscopic examination was made according to FAB criteria. Results From January 1998 to December 2007, 1152 patients had initial hematologic malignancies. Among them, 269 cases of acute lymphoblastic leukemia (ALL) accounted for 23.35%; 409 cases of acute non-lymphocytic leukemia (ANLL) accounted for 35.5%; 161 cases of chronic myeloid leukemia (CML) accounted for 13.98%; chronic lymphocytic leukemia ), 18 cases (1.56%), 5 cases of plasma cell leukemia (PCL), accounting for 0.43%, 91 cases of multiple myeloma (MM), accounting for 7.90%, 141 cases of myelodysplastic syndrome (MDS) accounting for 12.24% The other 58 cases, accounting for 5.03%. In the 0-9 age group, ALL accounted for 67.63% (94/139) in the first place. The incidence of ANLL was 53.29% (89/167) in the age group of 30-39 years. The incidence of CML was significantly increased in patients over 20 years old. The incidence of CLL The incidence of MM and MDS was significantly increased in patients over the age of 40, with the highest prevalence in the group of> 60 years old, accounting for 60.44% (55/91) and 57.45% (81/141), respectively. Conclusion Hematological malignancies have been increasing in recent years.
