目的:探讨阴道微生态状况检测分析在妇科门诊分泌物检查中的临床应用价值。方法:分析2014年10月—2014年12月青海红十字医院妇科门诊4 000例因不同症状就诊患者的阴道微生态评价(过氧化氢、唾液酸苷酶、白细胞酯酶、β-葡萄糖醛酸苷酶和凝固酶及阴道分泌物常规检查)检测结果。结果:4 000例被检查者平均年龄为(38±0.6)岁。其中阴道微生态正常占3.2%(128/4 000);阴道微生态失调者占96.8%(3 872/4 000);后者包括未明确感染的阴道微生态失调患者占21.23%(849/4 000);明确感染的阴道微生态失调患者占75.58%(3 023/4 000);其中细菌性阴道病(BV)患者占35.65%(1 426/4 000);外阴阴道假丝酵母菌病(VVC)患者占18.13%(725/4 000);需氧菌性阴道炎(AV)患者占9.63%(385/4 000);滴虫阴道炎(TV)患者占12.18%(487/4 000);混合感染患者占11.88%(475/4 000);优势菌群异常患者占77.05%(3 082/4 000)。结论:妇科门诊中阴道微生态失调患者构成比最高;在明确感染的患者中细菌性阴道病构成比最高;因此阴道微生态评价成为阴道感染性疾病诊断首选检测方法,在临床诊疗实践中具有重要意义。
Objective: To investigate the clinical value of vaginal microecological examination in gynecological clinic secretions. Methods: The vaginal microecological evaluation (hydrogen peroxide, neuraminidase, leukocyte esterase, β-glucuronic acid) of 4 000 gynecological clinics in Qinghai Red Cross Hospital from October 2014 to December 2014 was analyzed. The prevalence of vaginal microecology was 3.2% (128/4 000), vaginal microbiological disorders accounted for 96.8% (3872/4 000), and the latter including 21.23% (849/4) of patients with vaginal dysbiosis without clear infection 000); 75.58% (3 023/4 000) of patients with vaginal micro-organism infection were clearly infected; 35.65% (1 426/4 000) patients had bacterial vaginosis (BV); vulvovaginal candidiasis VVC) accounted for 18.13% (725/4 000), aerobic bacterial vaginitis (AV) accounted for 9.63% (385/4 000), and trichomonas vaginitis (TV) accounted for 12.18% (487/4 000) ; Mixed infection accounted for 11.88% (475/4 000); dominant flora accounted for 77.05% (3 082/4 000). Conclusions: The ratio of patients with vaginal microecological disorders in gynecology clinic is the highest, while the constituent ratio of bacterial vaginosis is the highest among patients with definite infection. Therefore, the evaluation of vaginal microecology is the first choice for the diagnosis of vaginosis, which is important in the practice of clinical diagnosis and treatment significance.
