目的比较弹性髓内钉与外固定支架治疗小儿股骨干骨折的临床疗效。方法 2012年3月-2014年8月治疗小儿股骨干骨折共26例,其中弹性髓内钉14例,外固定支架12例,所有病例均为闭合复位,对两种内固定术后骨折愈合时间,术后并发症进行比较分析。结果全部患者均获得随访,时间6-24个月,平均12个月。弹性髓内钉组治疗小儿股骨骨折在骨折临床愈合和骨性愈合时间均短于外固定支架组(P<0.05)。外固定支架组钉道感染1例,骨折延迟愈合1例,再骨折1例;弹性髓内钉组钉尾激惹1例。结论弹性髓内钉治疗小儿股骨干骨折有很大优势;而对于高能量骨折及多发伤的病例,外固定支架不失为一种良好的选择;股骨远端和近端骨折尽量不使用弹性髓内钉固定。
Objective To compare the clinical effects of elastic intramedullary nails and external fixators in the treatment of pediatric femoral shaft fractures. Methods From March 2012 to August 2014, a total of 26 cases of pediatric femoral shaft fractures were treated, including 14 cases of elastic intramedullary nailing and 12 cases of external fixation. All cases were closed and reset. The fracture healing time Postoperative complications were compared. Results All patients were followed up for 6-24 months with an average of 12 months. Elastic nail group treatment of pediatric femoral fractures in clinical fracture healing and bony union were shorter than the external fixation group (P <0.05). In the external fixator group, 1 case was infected with nostril pathology, 1 case was delayed union and 1 case was fractured again. Conclusion Flexible intramedullary nail treatment of pediatric femoral shaft fractures have great advantages; and for high-energy fractures and multiple injuries cases, external fixation is a good choice; distal and proximal femoral fractures try not to use elastic intramedullary nail fixed.
