目的探究急性髓系白血病(acute myeloid leukemia,AML)患者外周血视黄醇结合蛋白(retinol-binding protein,RBP)表达水平及其与患者临床特征的相关性。方法分析AML患者、良性血液病患者及健康体检者外周血RBP表达水平及其与AML患者年龄、性别、FAB(French-American-British)分型、细胞遗传学及基因突变的相关性。结果 AML患者外周血RBP表达水平明显低于良性血液病患者及健康对照组。M及M患者RBP表达水平与白细胞计数呈负相关,R分别4 5为-0.499与-0.412,P分别为0.036与0.029。APL(acute promyelocytic leukemia)患者RBP表达水平明显高于M(u=-2.085,P=20.037)、M(u=-3.085,P=0.002)及M(u=-2.249,P=0.024)患者,而其他FAB各亚型间RBP表达水平无明显差异(全4 5部P>0.05)。老年AML患者RBP表达水平明显高于中年(t=2.572,P=0.011)及儿童患者(t=2.284,P=0.049)。FLT3-ITD突变患者RBP表达水平明显低于无突变者(u=-2.173,P=0.030)。结论 RBP不仅可作为AML患者的营养指标之一,其表达水平及其与AML患者临床特征的相关性还可能为AML的优化治疗提供有价值的参考。
Objective To investigate the expression of retinol-binding protein (RBP) in peripheral blood of patients with acute myeloid leukemia (AML) and its correlation with clinical features. Methods The expression of RBP in peripheral blood of patients with AML, benign blood diseases and healthy controls and its association with age, sex, FAB (French-American-British) classification, cytogenetics and gene mutation were analyzed. Results The expression level of RBP in peripheral blood of AML patients was significantly lower than that of benign blood diseases and healthy controls. The expression of RBP in M and M patients was negatively correlated with leukocyte count, R 4 5 -0.499 and -0.412, P 0.036 and 0.029, respectively. The expression of RBP in patients with APL was significantly higher than those in patients with M (u = -2.085, P = 20.037), M (u = -3.085, P = 0.002) and M (u = -2.249, There was no significant difference in RBP expression levels between other subtypes of FAB (all 45 P> 0.05). RBP expression was significantly higher in elderly AML patients than in middle-aged patients (t = 2.572, P = 0.011) and children (t = 2.284, P = 0.049). The expression of RBP in patients with FLT3-ITD mutation was significantly lower than those without mutation (u = -2.173, P = 0.030). Conclusion RBP not only can be used as a nutritional indicator of AML patients, but its expression level and its correlation with clinical features of AML patients may also provide valuable reference for the optimal treatment of AML.
