目的观察盐酸羟考酮注射液(简称羟考酮)复合丙泊酚在无痛人工流产术(简称人流术)中的麻醉效果。方法要求行无痛人流术的早孕患者90例,ASAⅠ或Ⅱ级,随机数字分为Q1组(羟考酮0.05 mg/kg)、Q2组(羟考酮0.1 mg/kg)和Q3组(羟考酮0.15 mg/kg),各30例,三组均缓慢静脉注射,2 min后分别给予丙泊酚2 mg/kg。观察三组T1(麻醉前)、T2(睫毛反射消失时)、T3(手术开始时)、T4(手术结束时)、T5(清醒时)的收缩压(SBP)、心率(HR)、血氧饱和度(Sp O2),记录术后宫缩痛情况、患者满意度并比较不良反应。结果与T1相比,T2~T5,Q3组Sp O2明显降低(P<0.05);T2~T4,Q2、Q3组SBP明显降低、HR明显减慢(P<0.05);T2~T3,Q1组SBP明显降低、HR明显减慢;与前两组相比,Q3组SBP、Sp O2在T3明显降低(P<0.05);与Q2组相比,Q1组HR在T3明显增快,Q3组HR在T3、T4明显减慢(P<0.05)。Q1组体动发生率高,Q3组呼吸抑制、恶心呕吐发生率高,差异有统计学意义(P<0.05)。结论盐酸羟考酮注射液0.1 mg/kg复合丙泊酚用于无痛人流术,麻醉效果确切,不良反应少,患者满意度高。
Objective To observe the anesthetic effects of oxycodone hydrochloride injection (oxycodone) combined with propofol in painless artificial abortion (abbreviated abortion). Methods 90 cases of early pregnancy with painless abortion were randomly divided into Q1 group (oxycodone 0.05 mg / kg), Q2 group (oxycodone 0.1 mg / kg) and Q3 group Oxycodone 0.15 mg / kg), 30 in each. All three groups were given slow intravenous injection and propofol 2 mg / kg after 2 min. The three groups were observed before T1 (before anesthesia), T2 (when eyelashes disappeared), T3 (at the beginning of surgery), T4 (at the end of surgery), SBP (SBP), heart rate Saturation (Sp O2), records of postoperative uterine pain, patient satisfaction and more adverse reactions. Results The levels of SpO2 in T2 ~ T5 and Q3 groups were significantly lower than those in T1 groups (P <0.05). The SBP in T2 ~ T4, Q2 and Q3 groups were significantly decreased and HR significantly decreased (P <0.05) Compared with the former two groups, SBP and Sp O2 in Q3 group decreased significantly at T3 (P <0.05). Compared with Q2 group, HR in Q1 group was significantly increased at T3, while Q3 group’s HR At T3, T4 was significantly slower (P <0.05). There was a high incidence of body movement in Q1 group, and a high incidence of respiratory depression and nausea and vomiting in Q3 group (P <0.05). Conclusion Oxycodone hydrochloride injection 0.1 mg / kg propofol combined with propofol for painless abortion, the exact effect of anesthesia, fewer adverse reactions, patients with high satisfaction.
