目的探讨单纯泪道YAG激光成形术与泪道YAG激光成形联合鼻泪管逆行植入球头硅胶管手术在治疗鼻泪管阻塞方面的不同疗效,为基层临床眼科筛选一种安全简便、疗效可靠、低创伤的治疗方法。方法选择2005年1月至2007年6月就诊的鼻泪管阻塞患者125例152眼,将其随机分为A、B两组,A组采用单纯泪道YAG激光成形术,B组采用泪道YAG激光成形联合鼻泪管逆行植入球头硅胶管手术治疗。两组均于术后前3 d,每天以氧氟沙星注射液及地塞米松行泪道冲洗1次,以后每周冲洗1次至术后1个月。按疗效标准对两组治疗效果进行分析、比较。结果A组60例75眼,1个月时总有效率96%,3个月时总有效率85.33%,6个月时总有效率80.56%,第6个月时失访3眼。B组65例77眼,1个月时总有效率100%,3个月时总有效率96.1%,6个月时总有效率93.15%,第6个月时失访4眼。结论泪道激光成形联合鼻泪管逆行植入球头硅胶管较单纯泪道激光成形术疗效较为明显,尤其是远期疗效。采用泪道YAG激光成形联合鼻泪管逆行植入球头硅胶管方法,充分利用了激光治疗及球头硅胶管植入的各自优势,进一步降低了疾病复发的可能性,达到了双重的功效,值得基层临床广泛推广开展。
Objective To investigate the curative effect of simple lacrimal YAG laser lacrimal surgery and lacrimal YAG laser combined with retrograde nasolacrimal duct implantation on the treatment of nasolacrimal duct obstruction. The aim of this study is to screen a safe and simple, reliable curative effect for primary ophthalmic screening Low-trauma treatment. Methods A total of 152 eyes of 125 patients with nasolacrimal duct obstruction who were treated from January 2005 to June 2007 were randomly divided into A and B groups. Group A received lacrimal YAG laser angioplasty. Group B received lacrimal duct YAG laser combined with nasolacrimal duct retrograde ball tube silicone tube surgery. Both groups were in the first 3 days after surgery, ofloxacin injection and dexamethasone lacrimal duct flush 1, after a week of flushing to 1 month after surgery. According to the efficacy of the standard treatment of two groups were analyzed, compared. Results A group of 60 cases 75 eyes, the total effective rate was 96% at 1 month, the total effective rate was 85.33% at 3 months, the total effective rate was 80.56% at 6 months and lost to 3 eyes at 6 months. The total effective rate was 100% at 1 month in group B, 96.1% at 3 months, 93.15% at 6 months and 4 eyes lost at 6 months. Conclusion The lacrimal duct laser combined with nasolacrimal duct retrograde ball capillaries is more effective than pure laser lacrimal ductoplasty, especially for long-term curative effect. Using lacrimal YAG laser combined with retrograde nasolacrimal duct ball-head silicone tube method, take full advantage of the laser treatment and ball-head silicone tube implantation of their respective advantages and further reduce the possibility of disease recurrence, to achieve a dual effect, Worthy of extensive promotion of primary clinical work.
