目的:观察白芍总苷对非酒精性脂肪性肝病大鼠网膜素、TLR4蛋白的调控作用。方法:取高果糖-高脂诱导非酒精性脂肪性肝病大鼠模型,将模型大鼠随机分为模型组,水飞蓟宾组(200mg/kg),二甲双胍组(200mg/kg),白芍总苷组(200mg/kg,100mg/kg),另设正常对照组。用药6周后观察各组大鼠血清天门冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、空腹血糖(FBG)、餐后2小时血糖(2h BG)、胰岛素(Fins)、游离脂肪酸(FFA)、甘油三酯(TG)、胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)及网膜素(Omentin)水平,并计算肝脏指数和胰岛素抵抗指数(HOMA-IR);以蛋白免疫印迹法(Western bolt)检测肝组织Omentin、Toll样受体4(TLR4)蛋白的活性。结果:与模型组比较,白芍总苷组(200mg/kg,100mg/kg)2h BG、Fins、HOMA-IR、LDL-C、TG、TC、FFA、ALT、AST均明显降低,白芍总苷组(200mg/kg,100mg/kg)在拮抗胰岛素抵抗、调节血脂血糖异常,改善肝功能方面有较显著的作用,白芍总苷组(200mg/kg,100mg/kg)均可下调TLR4、网膜素的表达。结论:TLR4介导非酒精性脂肪性肝病的炎症反应及纤维化,网膜素通过抑制TLR4的过高表达而控制机体炎症损伤,减轻非酒精性脂肪性肝病向肝纤维化、肝硬化发展的进程;白芍总苷通过下调TLR4蛋白的表达而抑制炎症信号通路的转导,阻止炎症的发生发展,同时拮抗胰岛素抵抗而起到保护肝脏的作用。

Results: Compared with the model group, the content of BG, Fins, HOMA-IR, LDL-C, TG, TC, FFA, ALT and AST in the total glycosides of paeony (200mg / kg, 100mg / Glycosides (200mg / kg, 100mg / kg) had a significant effect on antagonizing insulin resistance, regulating blood lipid and glucose and improving liver function. Overexpression of TLR4 can control the inflammatory injury and reduce the development of hepatic fibrosis and cirrhosis in patients with non-alcoholic fatty liver disease by inhibiting the overexpression of TLR4 Total glycosides of paeony can inhibit the development of inflammation by down-regulating the expression of TLR4 protein and inhibit the transduction of inflammatory signaling pathway, and at the same time antagonize insulin resistance and play a role in protecting the liver.
