[目的]探讨肠内营养应用在食管癌手术患者中对营养状况与胃肠道耐受的影响及临床价值。[方法]选择我院行手术治疗的食管癌患者90例,采取随机数字表法分为观察组和对照组,对照组术后给予全肠外营养,观察组在对照组基础上联合肠内营养治疗,记录2组患者术后营养状况。[结果]观察组干预后体重指数(21.36±0.45)kg/m2,血红蛋白浓度(127.83±6.87)g/L,血清白蛋白(44.62±5.73)g/L;对照组干预后体重指数(20.16±0.13)kg/m2,血红蛋白浓度(115.09±3.21)g/L,血清白蛋白(39.05±2.18)g/L,2组比较差异有统计学意义(t=17.1858、11.2704、6.0947,P<0.05)。观察组肛门排气时间(1.89±0.56)d,排便时间(2.34±0.65)d,住院时间(13.28±3.05)d,并发症发生率4.44%;对照组肛门排气时间(2.76±1.41)d,排便时间(3.54±1.78)d,住院时间(20.87±5.85)d,并发症发生率17.78%,2组比较差异有统计学意义(χ2或t=3.8468、4.2480、7.7175、4.0500,P<0.05)。[结论]食管癌手术患者术后应用肠内营养能够改善患者营养状态,维护并促进胃肠道功能,安全可行,值得在临床大力推广使用。
[Objective] To investigate the effect of enteral nutrition on nutritional status and gastrointestinal tolerance in esophageal cancer patients and its clinical value. [Methods] Ninety patients with esophageal cancer undergoing surgery in our hospital were selected and divided into observation group and control group by random number table. The control group was given total parenteral nutrition after operation. The observation group was combined with enteral nutrition Treatment, record 2 groups of patients nutritional status. [Results] Body mass index (21.36 ± 0.45) kg / m2, hemoglobin concentration (127.83 ± 6.87) g / L and serum albumin (44.62 ± 5.73) g / L in the observation group after intervention were 20.16 ± 0.13) kg / m2, hemoglobin concentration (115.09 ± 3.21) g / L and serum albumin (39.05 ± 2.18) g / L respectively. There was significant difference between the two groups (t = 17.1858,11.2704,6.0947, P <0.05) . The anal exhaust time (1.89 ± 0.56) d, defecation time (2.34 ± 0.65) d, hospital stay (13.28 ± 3.05) d and the incidence of complications in the observation group were 4.44% , Defecation time (3.54 ± 1.78) d, hospitalization time (20.87 ± 5.85) d, complication rate was 17.78%, there was significant difference between the two groups (χ2 or t = 3.8468,4.2480,7.7175,4.0500, P <0.05 ). [Conclusion] The application of enteral nutrition after operation in patients with esophageal cancer can improve the nutritional status of patients, maintain and promote the gastrointestinal function, and is safe and feasible. It is worth to be widely used clinically.
