目的评价改良牙槽嵴顶结缔组织转瓣技术在上颌前牙种植美学修复中的应用效果。方法选取20例上前牙单颗缺失种植复诊行二期手术且术前评估种植区唇侧软组织丰满度不足的患者作为实验组,二期手术同时行改良牙槽嵴顶结缔组织转瓣术,选取二期术前、术后2周、1个月、6个月及1年为观测时间点,记录唇侧软组织丰满度评分、唇侧软组织增量高度、牙间乳头相对高度、粉色美学指数。同时收集20例符合纳入标准但未行转瓣术病例作为对照组并记录以上数据。采用t检验、重复测量的方差分析等统计学方法对比分析数据。结果唇侧软组织丰满度评分及唇侧软组织丰满度测量结果显示,实验组术后各时间点软组织丰满度均较术前有增加(测量值T1-T0为0.3;T2-T0为0.27;T3-T0为0.26;T4-T0为0.26),且与对照组相比效果明显(P<0.001),短期及长期效果均良好;牙间乳头相对变化结果及PES评分显示实验组与对照组间无统计学差异(P>0.05)。结论种植二期使用改良牙槽嵴顶结缔组织转瓣技术,能有效增加唇侧软组织丰满度且稳定性良好,不会造成牙间乳头高度及PES评分的降低,适宜在种植修复软组织美学领域推广使用。
Objective To evaluate the application effect of modified alveolar ridge connective tissue rotation technique in amending the maxillary anterior teeth. Methods Twenty cases of single upper anterior teeth missing compound implants in the second line of operation and preoperative evaluation of the labial soft tissues in implant area were selected as the experimental group. The preoperative and postoperative 2 weeks, 1 month, 6 months and 1 year were selected as the observation time point. The labial soft tissue fullness score, the labial soft tissue increment height, the relative height of the interdental papillae, the pink aesthetics index . At the same time, 20 patients who met the inclusion criteria but did not undergo revascularization were selected as the control group and the above data were recorded. Using t test, repeated measures of variance analysis and other statistical methods to compare the data. Results The labial soft tissue plumpness score and the labial soft tissue plumpness measurement results showed that the soft tissue plumpness in the experimental group was significantly higher than that before the operation (T1-T0 was 0.3; T2-T0 was 0.27; T3- T0 was 0.26; T4-T0 was 0.26), and the effect was obvious compared with the control group (P <0.001), the short-term and long-term effects were good; the relative change of interdental papillae and PES score showed no statistical difference between the experimental group and the control group Learning difference (P> 0.05). Conclusions The technique of modified alveolar-crown connective tissue revascularization in the second stage of implantation can effectively increase the labial soft tissue fullness and good stability without reducing the interdental papilla height and PES score and is suitable for the promotion of soft tissue aesthetics in implant repair use.
