目的评价99mTc-HL91乏氧显像在肺癌诊断中的临床价值,并探讨其与预后的关系。方法对21例肺部原发性恶性肿瘤和10例对照者行99mTc-HL91乏氧显像,患者在注射99mTc-HL91后2 h行断层显像,对显像阳性患者利用感兴趣区(ROI)技术分别勾画肿瘤(T)与对侧相应部位(N),计算T/N比值。结果99mTc-HL91乏氧显像肺部病灶显影灵敏度为85.7%(18/21),特异性为90.0%(9/10),准确度为87.1%(27/31);淋巴结显影灵敏度64.7%(11/17)。10例对照者肺部显像中1例炎症伴肺不张为阳性,余均为阴性。随访好转组与死亡组99mTc-HL91显像肺部病灶T/N比值分别为2.07±0.53与3.14±0.76(t=2.52,P<0.05)。结论99mTc-HL91乏氧显像诊断肺癌及其淋巴结转移具有较高的临床应用价值,并对患者的预后判断具有一定的意义。
Objective To evaluate the clinical value of 99mTc-HL91 hypoxia imaging in the diagnosis of lung cancer and to explore its relationship with prognosis. Methods Twenty-one patients with primary pulmonary malignancy and 10 controls underwent 99mTc-HL91 hypoxia imaging. The patients underwent 99mTc-HL91 hypoxia imaging 2 h after injection of 99mTc-HL91. The ROI ) Technique were delineated tumor (T) and the corresponding contralateral contralateral (N), calculate the T / N ratio. Results The sensitivity of 99mTc-HL91 hypoxic imaging in developing lung lesions was 85.7% (18/21), specificity was 90.0% (9/10), accuracy was 87.1% (27/31), lymph node imaging sensitivity was 64.7% 11/17). One case of inflammation and pulmonary atelectasis was positive in 10 cases of lung imaging. All the others were negative. The T / N ratios of lung lesions in 99mTc-HL91 imaging group were 2.07 ± 0.53 and 3.14 ± 0.76 (t = 2.52, P <0.05), respectively. Conclusion 99mTc-HL91 hypoxia imaging has a high clinical value in diagnosis of lung cancer and lymph node metastasis, and has certain significance for the prognosis of patients.
