目的:血栓栓塞是二尖瓣狭窄(MS)常见而严重的并发症,然而血栓栓塞的发病机理尚未完全清楚,本研究检测MS病人止血系统的多项指标,旨在证实MS血栓前状态(PTS)的存在并探讨其发生机理;方法:通过测定12例MS患者的反映内皮损伤(VMF)、血小板激活(GMP-140)、凝血酶产生(FPA)和纤溶状态(D-D)等生化标志物的血浆水平;结果:VWF、FPA和D-D血浆浓度明显高于正常对照组134.75%±25.78%、(10.05±5.44)mg/L、(0.46±0.15)g/L,P<0.01~0.05);结论:MS病人在止血系统的多个环节发生异常,存在PTS;合并房颤的MS病人止血系统异常尤为显著;PTS与左心房内径或二尖瓣口面积无关。
PURPOSE: Thromboembolism is a common and serious complication of mitral stenosis (MS). However, the pathogenesis of thromboembolism is not fully understood. This study examined multiple indicators of the hemostatic system in patients with MS to confirm the prethrombotic state of MS (PTS ), And to explore its mechanism of action.Methods: The biochemical tests of VMF, GMP-140, FPA and D-D in 12 patients with MS were performed. Plasma levels of VWF, FPA and D-D were significantly higher than those of the control group (134.75% ± 25.78%, (10.05 ± 5.44) mg / L, (0.46 ± 0.15) g / L, P <0.01 ~ 0.05) .Conclusion: MS patients have abnormalities in various parts of the hemostatic system with PTS present. The abnormalities of the hemostatic system in MS patients with atrial fibrillation are particularly significant. PTS And left atrial diameter or mitral orifice area has nothing to do.
