目的探讨食管癌肿瘤标志物在食管癌细胞中的表达及临床意义。方法选取2013年4月-2015年12月驻马店市中心医院收治的65例食管癌患者作为观察组,另选取63例同期健康体检者作为对照组。观察组均行胸腹腔镜食管癌根治术治疗,并根据治疗后复发情况分组。检测比较各组血清癌抗原19-9(CA19-9)、癌胚抗原(CEA)、癌抗原724(CA724)及鳞状上皮细胞癌相关抗原(SCC)等血清肿瘤标志物水平及诊断结果。结果观察组血清CA19-9、CEA、CA724及SCC水平均高于对照组,差异有统计学意义(均P<0.05)。血清各肿瘤标志物联合检测准确率为89.84%(115/128)、灵敏性为83.08%(54/65)、特异性为96.83%(61/63),均高于各指标单一检测;联合检测误诊率为3.17%(2/63)、漏诊率为16.92%(11/65),均低于各单一检测;以上差异均有统计学意义(均P<0.05)。复发组血清CA19-9、CEA、CA724、SCC水平均高于未复发组,差异均有统计学意义(均P<0.05)。结论血清癌抗原19-9、癌胚抗原、癌抗原724及鳞状上皮细胞癌相关抗原联合检测诊断准确率及灵敏性、特异性高,漏诊、误诊率低,可作为临床鉴别诊断早期食管癌及术后效果评估的依据。
Objective To investigate the expression and clinical significance of esophageal cancer tumor markers in esophageal cancer cells. Methods From April 2013 to December 2015, 65 patients with esophageal cancer admitted to Zhumadian Central Hospital were selected as the observation group, and 63 healthy people were selected as the control group. The observation group underwent thoracoscopic laparoscopic radical resection of esophageal cancer and grouped according to the recurrence after treatment. Serum tumor markers such as serum tumor antigen 19-9 (CA19-9), carcinoembryonic antigen (CEA), cancer antigen 724 (CA724) and squamous cell carcinoma-associated antigen (SCC) were detected and compared. Results The levels of serum CA19-9, CEA, CA724 and SCC in the observation group were significantly higher than those in the control group (all P <0.05). The accuracy of the combined detection of serum tumor markers was 89.84% (115/128), the sensitivity was 83.08% (54/65) and the specificity was 96.83% (61/63), which were all higher than the single detection of each index. Combined detection The misdiagnosis rate was 3.17% (2/63) and the rate of misdiagnosis was 16.92% (11/65), which were all lower than those of single test. The above differences were statistically significant (all P <0.05). The serum levels of CA19-9, CEA, CA724 and SCC in the recurrent group were significantly higher than those in the recurrent group (all P <0.05). Conclusion The combined detection of serum tumor antigen 19-9, carcinoembryonic antigen 724 and squamous cell carcinoma antigen has high diagnostic accuracy and sensitivity, high specificity, missed diagnosis and low misdiagnosis rate, which can be used as a differential diagnosis in early esophageal cancer And the basis for the evaluation of postoperative effects.
