目的 探讨新生儿缺氧缺血性脑病 (NHIE)内分泌代谢变化及其与 NHIE病情轻重的相互关系。方法 采用放射免疫方法测定 5 9例足月 NHIE患儿三碘甲状腺原氨酸 (T3)、甲状腺素 (T4)、游离 T3(FT3)、游离 T4(FT4)、促甲状腺素 (TSH)、促肾上腺皮质激素 (ACTH )、皮质醇 (C)、胰岛素 (INS)、胰高血糖素 (GC)、降钙素(CT)、甲状旁腺素 (PTH)、生长激素 (GH)浓度 ,并测血糖 (BS)及血钙。结果 NHIE存在内分泌代谢紊乱 ,表现为低 T3综合征 ,C升高 ,血钙降低 ,轻度 NHIE BS略升高 ,中、重度 NHIE BS可升高或降低 ,中、重度 NHIE BS升高或降低 ,ACTH、GC、CT升高 ,中、重度 NHIE BS病情愈重 ,变化愈明显。结论 T3、T4、C、BS浓度可作为临床判断NHIE病情轻重的参考指标。
Objective To investigate the changes of endocrine and metabolic changes in neonates with hypoxic-ischemic encephalopathy (NHIE) and their relationship with the severity of NHIE. Methods Radioimmunoassay was used to detect the levels of triiodothyronine (T3), thyroxine (T4), free T3 (FT3), free T4 (FT4) and thyrotropin (TSH) (ACTH), cortisol (C), insulin (INS), glucagon (GC), calcitonin (CT), parathyroid hormone (PTH) and growth hormone (GH) Blood sugar (BS) and blood calcium. Results There was an endocrine and metabolic disorder in NHIE with low T3 syndrome, elevated C, decreased serum calcium, mild mild NHIE BS, moderate or severe NHIE BS, increased or decreased moderate and severe NHIE BS, , ACTH, GC, CT increased, moderate and severe NHIE BS more serious changes in the more obvious. Conclusion The concentrations of T3, T4, C and BS can be used as a reference index to judge the severity of NHIE in clinic.
