目的探讨糖尿病病程对煤工尘肺合并糖尿病患者肺通气功能及血气的影响。方法选择煤工尘肺合并糖尿病患者80例作为观察对象,根据糖尿病病程分为糖尿病病程≤10年组33例(CWP+DM病程≤10年组)和糖尿病病程>10年组47例(CWP+DM病程>10年组);煤工尘肺患者80例作为对照组(CWP组)。对所有患者均行肺功能及血气分析检测。结果三组用力肺活量(FVC)、1 s用力呼气容积(FEV1)、用力呼气中段流量(MEF 25%~75%)比较,CWP+DM病程>10年组低于另外两组,差异有统计学意义(P<0.05);CWP+DM病程≤10年组低于CWP组,差异有统计学意义(P<0.05);三组FEV1/FVC%比较,无统计学意义(P>0.05)。动脉血氧分压(Pa O2)比较,CWP+DM病程>10年组低于另两组,差异有统计学意义(P<0.05);CWP+DM病程≤10年组低于CWP组,差异有统计学意义(P<0.05)。动脉血二氧化碳分压(Pa CO2)比较,CWP+DM病程>10年组高于另外两组,差异有统计学意义(P<0.05),CWP+DM病程≤10年与CWP组比较,差异无统计学意义(P>0.05)。结论煤工尘肺合并糖尿病患者肺通气功能损伤以及低氧血症随糖尿病病程延长而加重。
Objective To investigate the effect of diabetes duration on pulmonary ventilation and blood gas in coal workers with pneumoconiosis and diabetes mellitus. Methods According to the duration of diabetes, 33 patients with diabetes less than 10 years (CWP + DM course ≤ 10 years) and 47 patients with DM course> 10 years (CWP + DM Duration of> 10 years); coal workers pneumoconiosis 80 cases as a control group (CWP group). Arterial carbon dioxide partial pressure (PaCO2), CWP + DM duration> 10 years group was higher than the other two groups, the difference was statistically significant (P <0.05), CWP + DM duration ≤ 10 years compared with the CWP group, the difference was Statistical significance (P> 0.05). Conclusion The pneumoconiosis injury and hypoxemia in coal worker pneumoconiosis with diabetes mellitus aggravate with the prolongation of diabetes.
