在我国,医疗服务的资源基本上是直接或间接由行政部门配置。行政部门一是通过各种行政管制控制社会资源在医疗领域的配置,二是通过行业政策、财政政策在行业体系内根据医疗卫生机构的等级、所有制性质等进行资源分配。本文通过分析医疗行政部门的管制措施及其层层管制下的结果,揭示行政部门的管制不仅没有解决市场失灵,反而带来了政府失灵,降低了资源配置效率。并且通过构造一个资源分配的公平性评价方法和标准,直观地显示了行政部门的科层制配置机制下,医疗资源,尤其是财政医疗资金在省际和群体中配置的失衡和不公平的程度。
In our country, the resources of medical service are basically directly or indirectly allocated by the administrative department. First, the executive branch controls the allocation of social resources in the medical field through various administrative controls. Second, through industrial policies and fiscal policies, resources are allocated within the industrial system according to the level of medical and health institutions and the nature of ownership. By analyzing the control measures of the medical administrative department and the results under the layers of control, this article reveals that the control of the administrative departments has not only failed to solve the market failure, but has brought about the government failure and reduced the efficiency of resource allocation. And through the construction of a method and standard for fairness of resource allocation, it shows the imbalance and unfairness of medical resources, especially financial medical funds allocated in different provinces and groups under the administrative bureau-level allocation mechanism. .
