目的探讨老年2型糖尿病(T2DM)患者血25羟维生素D_3(VD_3)水平与早相胰岛素分泌的关系。方法选择老年DM前期40例为A组、新诊断T2DM 40例为B组、经治T2DM 50例为C组、老年健康者50名为对照(NC)组。ELISA法测定血VD_3,以△I_(30)/△G_(30)及HOMA-β反映早相胰岛素分泌功能。结果从NC到A、B及C组,随糖代谢紊乱的逐渐加重,VD_3水平逐渐降低(P均<0.05)。130例糖代谢异常患者随VD_3水平逐渐增高,早相胰岛素分泌功能逐渐好转(P均<0.05);反之,随早相胰岛素分泌功能逐渐好转,VD_3水平逐渐增高(P均<0.05)。回归分析显示,VD_3与早相胰岛素分泌功能呈正相关(P均<0.05)。结论老年T2DM患者存在VD_3缺乏,可能是导致其早相胰岛素分泌功能障碍的因素之一。
Objective To investigate the relationship between serum 25-hydroxyvitamin D3 (VD_3) and early phase insulin secretion in elderly patients with type 2 diabetes mellitus (T2DM). Methods Forty cases in the early stage of senile DM were selected as group A, 40 cases of newly diagnosed T2DM as group B, 50 cases of T2DM as group C and 50 healthy controls as control group. Serum VD_3 was measured by ELISA. The early phase insulin secretion was detected by △ I_ (30) / △ G_ (30) and HOMA-β. Results From NC to A, B and C groups, VD_3 level gradually decreased with the aggravation of glucose metabolism disorder (all P <0.05). The level of VD_3 gradually increased with the increase of VD_3 in 130 patients with abnormal glucose metabolism (P <0.05). On the contrary, the level of VD_3 gradually increased with the early phase insulin secretion (P <0.05). Regression analysis showed that VD_3 positively correlated with early phase insulin secretion (all P <0.05). Conclusion There is a lack of VD_3 in elderly patients with T2DM, which may be one of the factors leading to its early phase insulin secretion dysfunction.
