目的评估表观扩散系数(ADC)鉴别星形细胞瘤不同肿瘤组织及与细胞密度、病理分级的相关性。资料与方法采用Siemens1.5T超导型磁共振系统,对48例星形细胞瘤术前行常规MRI及扩散加权成像(DWI),其中Ⅰ、Ⅱ级23例,Ⅲ、Ⅳ级25例。分别计算不同肿瘤组织的ADC值。病理标本行HE染色,采用Image J1.33j分析每一标本的肿瘤细胞密度。用SPSS11.0对资料进行处理。结果星形细胞瘤不同肿瘤组织间及与正常脑实质的ADC值差异具有统计学意义(P<0.001);非强化的肿瘤实质与瘤周水肿区ADC值差异无统计学意义(P>0.05)。星形细胞瘤病理分级与强化肿瘤实质ADC值呈负相关,rs=-0.641(P<0.001);强化肿瘤实质的ADC值与肿瘤细胞密度呈负相关,r=-0.643(P<0.001)。结论星形细胞瘤ADC值能鉴别不同肿瘤组织,有助于评价肿瘤细胞密度及病理分级。
Objective To evaluate the relationship between apparent diffusion coefficient (ADC) and differential cell density and pathological grade in astrocytomas. Materials and Methods The Siemens 1.5T superconducting magnetic resonance system was used in 48 patients with astrocytoma. Conventional MRI and diffusion-weighted imaging (DWI) were performed preoperatively. There were 23 cases of grade Ⅰ and Ⅱ, and 25 cases of grade Ⅲ and Ⅳ. The ADC values of different tumor tissues were calculated. Pathological specimens were stained with HE and the tumor cell density of each specimen was analyzed using Image J1.33j. With SPSS11.0 data processing. Results The ADC value of astrocytoma between different tumor tissues and normal brain parenchyma had statistical significance (P <0.001). There was no significant difference in ADC value between non-enhanced tumor and peritumoral edema (P> 0.05) . There was a negative correlation between pathological grade of astrocytomas and ADC value of enhanced tumor (rs = -0.641, P <0.001). The ADC value of the enhanced tumor was negatively correlated with tumor cell density (r = -0.643, P <0.001). Conclusion The ADC value of astrocytoma can differentiate different tumor tissues and help to evaluate the tumor cell density and pathological grade.
