目的研究放疗联合不同剂量顺铂周疗在中晚期宫颈癌治疗中的效果。方法 75例中晚期宫颈癌患者,随机分为放疗组、小剂量组、大剂量组,每组25例。放疗组单纯进行放疗,小剂量组采用顺铂20 mg/m2的剂量联合放疗,大剂量组采用顺铂40 mg/m2的剂量联合放疗。以6周为周期,实验结束后评估治疗效果和毒性反应情况,对患者随访2年,计算2年生存情况。结果放疗组的有效率为40%,小剂量组的有效率为68%,大剂量组的有效率为72%,小剂量组和大剂量组的有效率均高于放疗组,差异均具有统计学意义(P<0.05);小剂量组的有效率与大剂量组比较差异无统计学意义(P>0.05)。放疗组2年生存率为64%,小剂量组2年生存率为80%,大剂量组2年生存率为76%,小剂量组2年生存率高于放疗组和大剂量组,但差异无统计学意义(P>0.05);放疗组2年生存率与大剂量组比较差异无统计学意义(P>0.05)。放疗组毒性反应发生率为8%,小剂量组毒性反应发生率为16%,大剂量组毒性反应发生率为44%,放疗组及小剂量组毒性反应发生率均低于大剂量组,差异均具有统计学意义(P<0.05);小剂量组的毒性反应发生率与放疗组比较差异无统计学意义(P>0.05)。结论放疗联合小剂量顺铂周疗可以显著提高中晚期宫颈癌治疗效果,减少毒性反应,提高患者生存率。
Objective To study the effect of radiotherapy combined with different doses of cisplatin in the treatment of advanced cervical cancer. Methods Seventy-five patients with advanced cervical cancer were randomly divided into radiotherapy group, low-dose group and high-dose group, with 25 cases in each group. The radiotherapy group was treated with radiotherapy only. The low-dose group was given cisplatin 20 mg / m2 combined with radiotherapy. The high-dose group was treated with cisplatin 40 mg / m2 combined with radiotherapy. The 6-week cycle, after the end of the experiment to assess the therapeutic effect and toxicity, follow-up of patients for 2 years to calculate the 2-year survival. There was no significant difference in the incidence of toxic reactions between low-dose group and radiotherapy group (P> 0.05). Conclusion Radiotherapy combined with low-dose cisplatin weekly treatment can significantly improve the treatment of advanced cervical cancer, reduce toxicity and improve patient survival.
