目的:观察糖尿病患者行白内障超声乳化吸出联合人工晶体植入术的临床效果。方法:56例(81只眼)糖尿病患者接受白内障超声乳化吸出联合人工晶体植入术,连续随机抽取年龄相当的非糖尿病患者56例(87只眼)作对照组。两组白内障超声乳化吸出联合人工晶体植入手术方法相同,植入光学部直径5.5毫米PMMA人工晶体。结果:糖尿病患者的81只眼中,术后矫正视力≥0.5者共63只眼(78%),非糖尿病患者的87只眼中,术后矫正视力≥0.5者为76只眼(87%)。两组经统计学处理,差异无显著性(P>0.05)。两组在术后并发症方面,差异亦无显著性(P>0.05)。但糖尿病患者中,术前瞳孔不能充分散大或术中瞳孔缩小的病例明显多于非糖尿病者,经统计学处理有显著性差异(P<0.05)。结论:糖尿病患者行白内障超声乳化吸出联合人工晶体植入术,可明显改善视力,在术后视力和手术并发症方面与一般患者无明显差异。说明超声乳化吸出术治疗糖尿病患者白内障是安全有效的,增殖性糖尿病视网膜病变和黄斑病变是影响术后视力的主要因素。小直径的人工晶体不影响术后眼底观察,并且为糖尿病视网膜病变的激光治疗提供了条件
Objective: To observe the clinical effect of phacoemulsification and intraocular lens implantation in diabetic patients. Methods: Fifty-six patients (81 eyes) with diabetes mellitus received phacoemulsification combined with intraocular lens implantation. Fifty-six consecutive non-diabetic patients (87 eyes) were randomly selected as the control group. Two groups of phacoemulsification combined with intraocular lens implantation surgery the same way, the implantation of optical diameter 5.5 mm PMMA intraocular lens. RESULTS: Of the 81 eyes with diabetes, 63 eyes (78%) had corrected visual acuity ≥ 0.5 and 76 eyes (87 eyes corrected visual acuity ≥ 0.5) of 87 eyes of non-diabetic patients %). There was no significant difference between the two groups (P> 0.05). There was no significant difference in postoperative complications between the two groups (P> 0.05). However, patients with diabetes mellitus, preoperative pupil can not be fully dispersed or intraoperative miosis cases were significantly more than non-diabetic patients, statistically significant difference (P <0.05). Conclusion: Cataract phacoemulsification combined with intraocular lens implantation in patients with diabetes mellitus can significantly improve the visual acuity. There is no significant difference in postoperative visual acuity and operative complications compared with the general patients. Description phacoemulsification cataract surgery in patients with cataract is safe and effective, proliferative diabetic retinopathy and macular degeneration is the main factor affecting postoperative visual acuity. Small diameter intraocular lens does not affect postoperative fundus observation, and provides the conditions for the laser treatment of diabetic retinopathy
