Objective To investigate the value of preoperative MRI in assessing the difficulty of microvascular decompression of hemifacial spasm. Methods The MRI findings of 105 patients with primary hemifacial spasm were surgically treated from April 2012 to September 2012. The sizes of responsible vessels, height of cerebellar bump, posterior fossa and subarachnoid space were observed and analyzed. The effect of morphology on the length of microsurgery operation. Results The size of the responsible vessel and posterior fossa morphology had a significant effect on the difficulty of the operation. Other factors may affect the difficulty of the operation. Conclusion The preoperative magnetic resonance imaging features are of great value in determining the degree of difficulty of microvascular decompression for hemifacial spasm.
