卫生站是营利性门诊中心的主要构成部分。卫生站医生的报酬直到1985年一律是计时工资制。在1985年中期,制定出一个新的付酬计划,企图以物质利益来刺激总收入的增加。这便是根据医生们每人创造的总收入多少来确定他们个人所获得奖金的多少。我们对在波士顿地区不同卫生站工作的15位医生做了调查。通过比较发现,同在新计划实施前后冬季的几个月中,医生们增加了病人每次就诊时实验室化验数字的23%;增加了病人每次就诊中x光检查数字的16%。扣除通货膨胀因素后每月总收入增长20%。增长的原因主要是这些月份病人就诊人数平均增加12%。七位医生因获得奖金而使个人所得报酬增加19%。由此我们可以得出一个结论,即个人收入的增加可以刺激医生们产生增加工作量的强烈愿望。尽管他们之中并非所有人都能从刺激中受益。
The health station is the main component of the profit clinic center. The doctor’s compensation at the health station was the hourly wage system until 1985. In the middle of 1985, a new compensation plan was created to try to stimulate the increase in total income with material benefits. This is based on the total amount of income each doctor creates. We investigated 15 doctors working at different health stations in the Boston area. Through comparison, it was found that during the months before and after the implementation of the new plan, doctors increased the number of laboratory tests at the time of each visit of the patient by 23%; and increased the number of x-rays of the patient at each visit by 16%. After deducting inflation, total monthly income increased by 20%. The increase was mainly due to an average increase of 12% in patient visits during these months. Seven doctors increased their personal remuneration by 19% due to their bonuses. From this, we can draw a conclusion that the increase in personal income can stimulate doctors to have a strong desire to increase the workload. Although not all of them can benefit from the stimulus.
