作者报告一例伴有眼球突出的鼻咽化学感受器瘤。患者女性,66岁,主诉左眼突出一年及流泪。阳性体征:左眼外突,左鼓膜内陷和中耳积液,鼻咽无异常。头颅侧位X线片显示颅底明显硬化,印象为颅底纤维性增生或脑膜瘤。三个月后发现鼻咽左侧有1厘米直径之肿块,病理报告为化学感受器瘤。X线放疗五周,共5000r,以后缩小放射野,又给1000r。放疗前左眼球突出度24毫米;右眼18毫米。放疗后左眼21毫米,右眼18毫米;鼻咽部肿物消失。放疗后九个月在鼻咽顶部又出现粘膜下肿胀,活检:纤维化和闭塞的血管围绕着分散的主细胞。中耳无肿物。治疗后随诊三年无症状。
The authors report one case of a nasopharyngeal chemoreceptor tumor with an outgrowth of the eyeball. The female patient, 66 years old, complained of left eye outstanding for one year and shedding tears. Positive signs: left eye outer protrusion, left tympanic membrane invagination and middle ear effusion, nasopharyngeal abnormalities. Lateral radiographs of the skull show that the base of the skull is hardened, with the impression of cranial base fibrosis or meningioma. Three months later, a 1-cm diameter lump was found on the left side of the nasopharynx. The pathology report was a chemoreceptor tumor. X-ray radiotherapy for five weeks, a total of 5000r, after reducing the radiation field, and give 1000r. Left eyeball protrusion 24 mm before radiotherapy; right eye 18 mm. 21 mm left eye and 18 mm right eye after radiotherapy; nasopharyngeal mass disappeared. On the top of the nasopharyngeal, submucosal swelling occurred again nine months after radiotherapy. Biopsy: fibrotic and occluded blood vessels surrounded the dispersed main cells. There is no mass in the middle ear. Follow-up after treatment for three years was asymptomatic.
