目的探讨桥小脑角区微小表皮样囊肿MR诊断的最佳方法。方法回顾分析7例肿瘤仅局限在桥小脑角区的微小表皮样囊肿患者的常规MRT1WI、T2WI及液体衰减反转恢复(FLAIR)、弥散加权成像(DWI)和平衡式快速梯度回波(B-FFE)的影像资料,病例均经手术和病理证实。结果7例表皮样囊肿患者中有6例在T1WI、T2WI肿瘤信号类似于脑脊液(CSF))信号,另1例均为细条样等信号;6例FLAIR图像中,3例表现为略高信号改变,1例为可疑混杂信号改变,2例则未显示;3例DWI图像均显示出不同于CSF的高信号;而7例患者B-FFE图像上均表现为以等到略高信号为主混杂信号改变,与桥小脑角池内表现为极高信号的CSF形成鲜明对比,同时7例患者包括2例肿瘤隐藏在桥小脑角池内未引起局部脑池形态改变的小肿瘤均清晰地显示了肿瘤与三叉神经之间的关系,与术中所见一致。结论B-FFE技术在桥小脑角区表皮样囊肿早期诊断中具有重要意义。
Objective To investigate the best method for MR diagnosis of cerebellar small epidermoid cyst. Methods The routine MRT1WI, T2WI and FLAIR, DWI and B-MRS were performed in 7 patients with small epidermoid cyst confined to the cerebellopontine angle region. FFE) images, the cases were confirmed by surgery and pathology. Results Six of the seven epidermoid cysts showed signal similar to cerebrospinal fluid (CSF) signals in T1WI and T2WI), while the other one showed signals of thin strips. In 6 FLAIR images, 3 showed slightly higher signal changes , One case of suspected mixed signal changes, two cases did not show; 3 cases of DWI images showed high signal different from CSF; and 7 cases of patients with B-FFE images were shown to wait until slightly higher signal-based promiscuous signal Change, in sharp contrast with CSF showing a very high signal in the cerebellopontine angle pool, while 7 of the 7 small tumors, including 2 tumors hidden in the cerebellopontine angle pool that did not cause a change in the morphology of the local cerebral pools, The relationship between nerves, consistent with the intraoperative findings. Conclusion The B-FFE technique is of great importance in the early diagnosis of cerebellopontine angle of the cerebellopontine zone.
