孙某,男,7岁.1992年2月诊.家长代述:患儿呕吐反复发作年余.约2～3个月一次,每次持续3～4天.轻时肌注药物,重则输液治疗方愈.曾做钡透和B超检查,上消化道及肝脾均正常.今晨起,.自述头疼、鼻塞,继则呕吐两次.呕吐物开始为少许胃溶物,后为痰液,非喷射性.伴纳差,大便干.查体:T36.8°C,精神欠佳,欲吐病容,巩膜无黄染,咽部轻度充血,舌质红苔黄厚,心肺正常,腹软,肝脾均未触及,神经系统检查无阳性体征,脉浮数有力.查尿酮体-+.诊断:西医:1、周期性呕吐.2、上感.中医:呕吐.证属素有湿热兼外感风邪.治宜清热燥湿,降逆止呕、佐以疏风解表.方以温胆汤加味:代赭石12克、茯苓、双花、连翘、竹茹各9克,陈皮、半夏、枳实、薄荷(后下)、防风各6克,黄连、生甘草各3克.3剂,水煎少量多次分服.一剂后呕吐止.三剂服完,大便通,黄苔转薄,外感证状亦尽.胃纳差,上方去黄连、代赭石、银花、薄荷、防风、枳实量减至3克,加炒谷芽9克,内金6克,继服5剂告愈.后随访半年未发.
