目的:探讨原发性高血压患者肱踝动脉脉搏波传导速度(brachial-ankle artery pulse wave velocity,baPWV)与心血管疾病危险因素的相关性。方法:监测297例原发性高血压患者清晨血压水平,同时检测其baPWV、血脂、血糖、同型半胱氨酸(homocysteine,HCY)、肾小球滤过率(eGFR)等指标,并进行颈动脉彩超检查。采用SAS 9.2对baPWV与清晨血压、血脂、血糖、HCY、颈动脉斑块、患者年龄进行相关性分析。采用Stepwise对baPWV的独立影响因素进行回归分析。结果:baPWV与患者年龄、清晨血压水平、低密度脂蛋白胆固醇(LDL-C)浓度、肾小球滤过率(eGFR)、颈动脉斑块的形成相关(P<0.05),与空腹血糖、HCY浓度无明显相关。回归分析显示,清晨血压水平、LDL-C浓度、颈动脉斑块的形成是baPWV的独立影响因素(P<0.05)。结论:原发性高血压患者清晨血压升高、高胆固醇血症与动脉硬化显著相关。
Objective: To investigate the relationship between brachial-ankle artery pulse wave velocity (baPWV) and cardiovascular risk factors in patients with essential hypertension. Methods: Morning blood pressure was measured in 297 essential hypertension patients. The baPWV, blood lipid, blood glucose, homocysteine (HCY) and glomerular filtration rate (eGFR) Arterial ultrasound examination. SAS 9.2 pairs of baPWV and early morning blood pressure, blood lipids, blood glucose, HCY, carotid plaque, patient age were analyzed. Regression analysis was performed using Stepwise independent factors of baPWV. Results: baPWV was associated with age, early morning blood pressure, LDL-C concentration, glomerular filtration rate (eGFR) and carotid artery plaque formation (P <0.05) HCY concentration was not significantly correlated. Regression analysis showed that early morning blood pressure, LDL-C concentration and carotid plaque formation were independent factors of baPWV (P <0.05). Conclusions: Morning hypertension in patients with essential hypertension, hypercholesterolemia and atherosclerosis were significantly associated.
