目的 分析2015-2016年四川省基本公共卫生服务项目中高血压患者接受健康管理服务后血压控制情况,为科学评判控制效果提供依据.方法 采取多阶段分层随机抽样方法,在全省分别抽取21个县(区)的42个乡镇卫生院(社区卫生服务中心)开展调查,在每个机构等距抽取高血压患者健康档案各10份开展电话调查,核查档案表单、个人信息、体检记录、随访信息等,根据国家基本公共卫生服务规范设定条件判断档案信息准确性和服务规范性,记录最后一次随访时的血压,并邀请患者到机构现场测量血压,对血压控制情况进行分析.结果 2015-2016年,四川省高血压患者档案记录血压控制率为84.13%,现场测量血压控制率为41.67%,差异有统计学意义(x2=12.889,P<0.01).不同服务规范性的档案记录血压控制率差异有统计学意义(x2 =22.61,P<0.01),不同服务规范性的现场测量血压控制率差异无统计学意义(x2 =0.54,P=0.46).不同满意度及不同年度的血压控制率差异均无统计学意义.结论 四川省高血压患者健康管理服务取得较好效果,需进一步探索控制效果评判机制.“,”Objective To explore the effect of community health management for hypertensive patients in primary public health service project in Sichuan province.Methods There were 42 health clinics or community health service center in 21 counties or cities extracted each year based on method of multistage stratified sampling during 2015-2016.For each institution 10 resident health records of hyperpietic were sampled for telephone survey to investigate the accuracy and standard ability of services for patients according to the national basic public health service specifications.Recorded the blood pressure of the last follow-up,and invited patients to the on-site blood pressure measuring.Results The control rate of blood pressure which was recorded from the last follow-up was 84.13%,and which was on-site measured was 41.67% (x2 =12.889,P <0.01).The control rate of blood pressure in normative services was different significantly from the last follow-up records (x2 =22.61,P <0.01).The control rate of blood pressure in normative services was no different from on-site measured (x2 =0.54,P =0.46).The satisfaction rate and control rate of blood pressure in different year was not significant different.Conclusion The quality of managed hypertensive patients was acceptable,and the effect evaluation of health management services for hypertension still should be explore further.
