[摘要] 目的 探討第一、二代抗精神病药物治疗首发精神分裂症患者脑源性神经营养因子(BDNF)的变化与疗效及认知功能的相关性研究。 方法 选择2018年8月~2019年8月在我院精神科住院的首发性精神分裂症患者100例,根据盲抽法分为研究A组(1代抗精神病药物)及研究B组(2代抗精神病药物),并选择同期健康受试者50例为对照组,比较临床总体印象量表(CGI)评分、血清BDNF水平、威斯康星卡片分类测验(WCST)评分,并分析血清BDNF水平与疗效及认知功能的相关性。 结果 治疗2、4、6、8周,与研究A组相比,研究B组CGI评分均较低,但差异无统计学意义(P>0.05);与治疗2周相比,两组治疗4、6、8周的CGI评分均较低,差异有统计学意义(P<0.05);治疗8周后,与研究A组相比,研究B组血清BDNF水平略高,但差异无统计学意义(P>0.05);治疗8周后,研究A、B组的血清BDNF水平均较入院时升高,差异有统计学意义(P<0.05);治疗8周后,两组WCST各项评分均较入院时改善(P<0.05),但研究A组与研究B组相比,差异无统计学意义(P>0.05);经Pearson相关性检验,血清BDNF水平与WCST完成分类数、正确应答数呈正相关(r>0,P<0.05);血清BDNF水平与WCST错误应答数、持续错误数、总应答数及治疗8周CGI评分呈负相关(r<0,P<0.05)。 结论 首发精神分裂症患者应用第一、二代抗精神病药物治疗效果均较好,可有效改善血清BDNF水平,且BDNF水平与疗效及认知功能相关。

[Abstract] Objective To explore the correlation between the changes of brain-derived neurotrophic factor(BDNF) and the efficacy and cognitive function in first-onset schizophrenia patients treated with first- and second-generation antipsychotics. Methods A total of 100 patients with first-onset schizophrenia who were admitted to the Psychiatric Department of our hospital from August 2018 to August 2019 were selected and divided into study group A(the first generation antipsychotic drugs) and study group B(the second generation antipsychotic drugs) based on blind drawing. 50 healthy subjects in the same period were selected as the control group. The clinical general impression scale(CGI) score, serum BDNF level, and wisconsin card sorting test(WCST) score were compared. The correlation between serum BDNF level and efficacy and cognitive function was analyzed.
