本文报告汗腺肿瘤 12 8例 ,其中良性占 91.40 % ,恶性占 8.6 0 % ;好发部位以头面部为多 (46 .0 9% )。病理分型透明细胞汗腺瘤 47例 (占良性汗腺瘤 40 .17% ) ,混合瘤 2 1例 ,其它各类型例数相继递减。本组任意抽出各类型汗腺肿瘤蜡块 47例 ,进行四种免疫组化检查 ,结果 EMA和 S10 0阳性者只限于混合瘤 ;Desmin在各类汗腺瘤中均呈阴性反应。而 NSE除混合瘤全部病例均阳性外 ,乳头状汗腺腺瘤 ,汗管瘤 ,透明细胞汗腺瘤的部分病例也呈阳性反应。提示 EMA阴性汗腺瘤可能与肿瘤的高度去分化有关 ;S10 0阳性为该肿瘤细胞具有向分泌细胞分化的潜能 ;NSE阳性细胞也许是弥漫性神经内分泌细胞在肿瘤组织内浸润 ,反映了机体对肿瘤的一种反应 ;小汗腺螺旋腺瘤旁 NSE、S10 0阳性的神经纤维增多 ,应该是该肿瘤疼痛的原因。
This article reported 128 cases of sweat gland tumors, of which benign accounted for 91.40%, malignant 8.60%; good hair and head and face area (46.9%). Pathological type clear cell sweat adenomas in 47 cases (40.17% of benign sweat gland adenomas), mixed tumors in 21 cases, the number of other types of cases decreased. In this group, 47 cases of sweat gland tumor blocks of any type were arbitrarily extracted, and four immunohistochemical examinations were performed. The results showed that EMA and S10 0 positive patients were limited to mixed tumors; Desmin was negative in all types of sweat adenomas. NSE was positive in all cases except for mixed tumors. Papillary sweat gland adenomas, syringomas, and clear cell sweat adenomas also showed positive reactions. It is suggested that EMA-negative sweat adenoma may be related to the high degree of dedifferentiation of tumor; S10 0 positive is that the tumor cell has the potential to differentiate into secretory cells; NSE-positive cells may be diffuse infiltration of neuroendocrine cells in the tumor tissue, reflecting the body’s tumor A reaction; the NSE, S10 0-positive nerve fibers adjacent to the hypoammoid hemangioblastoma increased, which should be the cause of the tumor pain.
