目的:分析骨髓细胞形态学及骨髓小粒结构在AA及不典型AA、MDS鉴别诊断中的应用价值。方法:对年龄在1~12岁的15例AA、6例不典型AA及9例MDS患儿的骨髓细胞形态及骨髓小粒结构进行研究分析。结果:AA患儿骨髓涂片增生程度及小粒结构中造血细胞比例明显低于MDS,差异有统计学意义(P<0.05);MDS患儿骨髓小粒中原始细胞比例明显高于其他两组(P<0.05),并出现多系统病态造血。结论:骨髓细胞形态学分析结合骨髓小粒细胞成分及结构分析在AA、不典型AA和MDS的鉴别诊断中具有重要应用价值。
OBJECTIVE: To analyze the clinical value of myeloid cell morphology and bone marrow microdomains in the differential diagnosis of AA and atypical AA and MDS. Methods: The morphology of bone marrow cells and the structure of bone marrow in 15 AA patients aged from 1 to 12 years, 6 atypical AA patients and 9 children with MDS were studied. Results: The degree of proliferation of bone marrow smears in AA children and the proportion of hematopoietic cells in small granular structures were significantly lower than those in MDS (P <0.05). The proportion of blasts in bone marrow of children with MDS was significantly higher than that in other two groups <0.05), and multiple system morbid hematopoiesis. CONCLUSION: The morphological analysis of bone marrow cells combined with the composition and structure of granulosa cells in bone marrow has important application value in the differential diagnosis of AA, atypical AA and MDS.
