目的:探讨超声乳化手术中眼压异常增高的机制。方法:总结11例术中发生眼压异常增高的病例观察及处理结果,分析其发生的原因及其机制。结果:①5例属因后囊有小破孔,灌注液迷流所致高眼压。②6例属CCC直径过小,在作水分离时发生。结论:①当后囊有小破孔,在玻璃体皮质成形性较好者有可能发生灌注液迷流,引起高眼压,须减低灌注压或改为囊外手术。②CCC直径不宜过小,水分离时不宜一次大量地推注液体。
Objective: To investigate the mechanism of abnormal elevation of intraocular pressure during phacoemulsification. Methods: 11 cases of abnormal intraocular pressure occurred during the observation and treatment of intraocular pressure, its causes and mechanism. Results: ① 5 cases of posterior capsule due to small holes, perfusion fluid caused by intraocular hypertension. ② 6 cases of CCC diameter is too small, occurred in the water for separation. Conclusions: ① When the posterior capsule has a small hole, there is a possibility that the fluid in the vitreous cortex may flow into the perfusion fluid and cause ocular hypertension. The perfusion pressure should be decreased or replaced by extracapsular surgery. CCC diameter should not be too small, water should not be a lot of time to push the liquid.
