目的:观察自拟汤剂治疗小儿支气管哮喘的临床疗效,同时对患儿的免疫指标进行测定。方法:筛选2012年8月~2014年7月来我院儿科就诊的支气管哮喘患儿94例,随机分到对照组和观察组中,每组各47例。在常规综合治疗的基础上,对照组患儿给予布地奈德气雾剂(200μg/次)雾化吸入,早晚各1次;观察组患儿则采用自拟中药汤剂,每日1剂,早晚2次分服,两组治疗均10天为1个疗程,连续观察6个疗程。将两组患儿治疗前后的炎性指标IL-4和FN-γ,免疫指标CD_4~+、CD_8~+及CD_4/CD_8进行比较分析,将两组的临床疗效进行比较。结果:观察组的炎性指标IL-4显著低于对照组(P<0.05),IFN-γ显著高于对照组(P<0.05);观察组免疫指标CD_8~+显著高于对照组(P<0.05),CD_4~+及CD_4/CD_8则显著低于对照组(P<0.05);治疗后对照组总有效率为83.0%;观察组总有效率为97.9%,两组之间具有显著性差异(P<0.05)。结论:自拟中药汤剂通过对T淋巴细胞的作用,促进T细胞亚群Th1/Th2的平衡,有效调节免疫功能,进而达到治疗小儿支气管哮喘的作用。
Objective: To observe the curative effect of self-made decoction on children’s bronchial asthma, and to measure the immune index of children. Methods: A total of 94 children with bronchial asthma who were admitted to our pediatric department between August 2012 and July 2014 were randomly divided into control group and observation group, with 47 cases in each group. On the basis of routine comprehensive treatment, the control group of children given budesonide aerosol (200μg / time) inhalation, morning and evening 1; the observation group of children with self-made Chinese medicine decoction, 1 day, 2 times morning and evening service, two treatment were 10 days for a course of treatment, continuous observation of 6 courses. The inflammatory markers IL-4 and FN-γ before and after treatment were compared between the two groups, and the immune indexes CD_4 ~ +, CD_8 ~ + and CD_4 / CD_8 were compared and analyzed. The clinical efficacy of the two groups were compared. Results: The inflammatory index IL-4 in the observation group was significantly lower than that in the control group (P <0.05), and IFN-γ was significantly higher than that in the control group (P <0.05). The immune index CD_8 ~ + in the observation group was significantly higher than that in the control group (P <0.05). The total effective rate of the control group was 83.0% after treatment and the total effective rate of the observation group was 97.9%. There was significant difference between the two groups Difference (P <0.05). Conclusion: The self-made Chinese medicine decoction can improve the balance of Th1 / Th2 in T lymphocyte subsets through T lymphocyte function and effectively regulate the immune function, thus achieving the therapeutic effect on bronchial asthma in children.
