目的 分析评价术前动脉灌注化疗对大肠癌生物学行为的影响 ,并探讨术前动脉灌注化疗对肿瘤细胞影响的作用机制。方法 2 8例 (同意入组研究 )大肠癌患者随机分成术前动脉灌注化疗组 (12例 )和对照组 (16例 )。术前动脉灌注化疗采用动脉造影技术 ,经肿瘤营养动脉给予 5 Fu10 0 0mg ,表阿霉素 6 0mg ,丝裂霉素 10mg。全部患者均接受手术治疗 ,切除标本分别行常规病理检查及免疫组织化学染色 (Ⅷ因子相关抗原染色 )测定大肠癌组织微血管密度 (MVD)。微血管密度计数采用Weidner方法分别计数癌中心、癌黏膜表面及癌旁组织的微血管数。结果 术前动脉灌注化疗组大肠癌组织中心、癌黏膜表面及癌旁组织的MVD分别为 40 .46± 7.0 6、5 2 .2 7± 18.40和 49.92± 8.15 ;对照组大肠癌组织中心、癌黏膜表面及癌旁组织的MVD分别为 46 .0 9± 12 .2 1、73.44± 2 2 .0 6和5 1.94± 12 .6 4。两组之间癌黏膜表面的MVD值差异有显著性 (P <0 .0 5 ) ,癌旁组织及癌中心组织MVD差异无统计学意义。结论 术前动脉灌注化疗可以降低大肠癌组织黏膜表面的微血管密度。
Objective To evaluate the effect of preoperative arterial infusion chemotherapy on the biological behavior of colorectal cancer and to explore the mechanism of preoperative arterial infusion chemotherapy on tumor cells. Methods Twenty-eight patients (consenting to enrollment study) were enrolled in this study. Patients with colorectal cancer were randomly divided into preoperative arterial infusion chemotherapy group (n = 12) and control group (n = 16). Preoperative arterial infusion chemotherapy using arteriography, the tumor nutrition artery given 5 Fu10 0mg, epirubicin 60mg, mitomycin 10mg. All patients underwent surgical treatment. The resected specimens were examined for the microvessel density (MVD) by routine pathological examination and immunohistochemical staining (Ⅷ factor-related antigen staining). Microvessel density count Weigner method were used to count the number of microvessels in cancer center, cancer mucosal surface and adjacent tissues. Results The MVD of colorectal cancer tissue, cancer mucosal surface and adjacent non-cancerous tissue before arterial infusion chemotherapy were 40.46 ± 7.0 6, 52.27 ± 18.40 and 49.92 ± 8.15, respectively. In the control group, the MVD The MVD of the mucosal surface and adjacent tissues were 46.9 ± 12.2, 17.44 ± 22.0.6 and 5 1.94 ± 12.6, respectively. There was a significant difference in MVD between the two groups (P <0.05). There was no significant difference in the MVD between adjacent tissues and cancerous tissues. Conclusion Preoperative arterial infusion chemotherapy can reduce the microvessel density on the mucosal surface of colorectal cancer.
