目的探讨细胞外基质金属蛋白酶诱导因子(EMMPRIN)、细胞外基质金属蛋白酶(MMP)-7表达与子宫内膜癌侵袭转移的关系。方法收集对照组(包括11例增生期子宫内膜,11例分泌期子宫内膜,11例单纯性增生子宫内膜,11例复杂性增生子宫内膜)、不典型增生组(51例)和子宫内膜癌组(82例)子宫内膜标本,应用免疫组化法测定标本中EMMPRIN和MMP-7的表达,采用SAS 6.12软件比较EMMPRIN和MMP-7阳性表达率在3组间的差异,并分析EMMPRIN和MMP-7蛋白表达与子宫内膜癌临床病理特征的关系。结果对照组、不典型增生组、子宫内膜癌组EMMPRIN和MMP-7的阳性表达率分别为9.09%(4/44)和6.82%(3/44)、43.13%(22/51)和41.17%(21/51)、59.76%(49/82)和61.00%(50/82),组间差异有统计学意义(P<0.05)。不同年龄、不同原发肿瘤大小的子宫内膜癌患者间EMMPRIN、MMP-7的表达差异无统计学意义,但不同组织学分级、淋巴结转移与否患者间EMMPRIN、MMP-7的表达差异有统计学意义(P<0.05)。Spearman等级相关性检验分析表明子宫内膜癌组织中EMMPRIN的表达和MMP-7的表达正相关(r=0.863,P<0.01)。结论 EMMPRIN和MMP-7与子宫内膜癌的发生、发展及侵袭转移有关,可能协同促进了子宫内膜癌的淋巴结转移。
Objective To investigate the relationship between the expression of extracellular matrix metalloproteinase inducer (EMMPRIN), extracellular matrix metalloproteinase (MMP) -7 and the invasion and metastasis of endometrial carcinoma. Methods The control group (11 cases with proliferative endometrium, 11 cases with secretory endometrium, 11 cases with simple proliferative endometrium and 11 cases with complicated proliferative endometrium), atypical hyperplasia group (51 cases) and In endometrial carcinoma group (82 cases), the expression of EMMPRIN and MMP-7 were detected by immunohistochemistry. The positive rates of EMMPRIN and MMP-7 were compared among the three groups using SAS 6.12 software, The relationship between the expression of EMMPRIN and MMP-7 protein and clinicopathological features of endometrial carcinoma was analyzed. Results The positive rates of EMMPRIN and MMP-7 in control group, atypical hyperplasia group and endometrial carcinoma group were 9.09% (4/44), 6.82% (3/44), 43.13% (22/51) and 41.17 % (21/51), 59.76% (49/82) and 61.00% (50/82), respectively. The differences between the two groups were statistically significant (P <0.05). The expression of EMMPRIN and MMP-7 in endometrial carcinoma patients of different age and different primary tumor size had no statistical significance, but statistics of the expression of EMMPRIN and MMP-7 among patients with different histological grade and lymph node metastasis Significance (P <0.05). Spearman rank correlation analysis showed that the expression of EMMPRIN in endometrial carcinoma was positively correlated with the expression of MMP-7 (r = 0.863, P <0.01). Conclusions EMMPRIN and MMP-7 are associated with the occurrence, development, invasion and metastasis of endometrial carcinoma, which may synergistically promote the lymph node metastasis of endometrial carcinoma.
