目的探讨类风湿关节炎合并潜伏性结核感染患者Treg细胞的表达及临床意义。方法将2015年1月至2016年1月于本院诊断为潜伏性结核感染的类风湿关节炎患者30例纳入观察组,并将本院同期无结核感染的类风湿关节炎患者30例纳入对照组。检测两组患者血清中Treg细胞及相关炎性因子水平,探讨Treg细胞在潜伏性结核感染中的临床意义。结果观察组患者Treg细胞水平[(4.36±1.12)%]明显低于对照组[(5.46±1.62)%](P<0.05),而Th17细胞和Th17/Treg细胞水平[(0.21±0.04)%,(0.25±0.07)%]均高于对照组[(1.01±0.19)%,(0.21±0.04)%](P<0.05)。观察组患者白介素(IL)-17水平[(19.65±2.36)pg/ml]明显高于对照组[(16.84±3.12)pg/ml](P<0.05),而IL-10和肿瘤坏死因子-α(TNF-α)水平[(4.93±0.42)pg/ml,(3.98±0.36)ng/ml]均明显低于对照组[(6.37±1.35)pg/ml,(4.93±0.42)ng/ml](P<0.05)。Pearson相关性分析显示,两组患者Treg细胞水平与Th17细胞和IL-17水平均呈负相关(r分别为-0.32、-0.54,P<0.05),而Treg细胞水平与IL-10和TNF-α水平均呈正相关(r分别为0.47、0.61,P<0.05)。结论类风湿关节炎合并潜伏性结核感染患者Treg细胞水平下降,Treg细胞水平与结核感染密切相关。
Objective To investigate the expression of Treg cells in patients with rheumatoid arthritis complicated with latent tuberculosis infection and its clinical significance. Methods Thirty patients with rheumatoid arthritis diagnosed as latent TB in our hospital from January 2015 to January 2016 were enrolled in the observation group and 30 patients with rheumatoid arthritis without tuberculosis in our hospital were included in the control group group. The levels of Treg cells and related inflammatory cytokines in serum of two groups were detected to explore the clinical significance of Treg cells in latent tuberculosis infection. Results The level of Treg cells in the observation group [(4.36 ± 1.12)%] was significantly lower than that in the control group [(5.46 ± 1.62)%] (P <0.05), while the levels of Th17 cells and Th17 / Treg cells were , (0.25 ± 0.07)%] were significantly higher than those in the control group (1.01 ± 0.19% vs 0.21 ± 0.04%, P <0.05). The levels of IL-17 in the observation group [(19.65 ± 2.36) pg / ml] were significantly higher than those in the control group [(16.84 ± 3.12) pg / ml], while the levels of IL-10 and tumor necrosis factor- (4.93 ± 0.42) pg / ml, (3.98 ± 0.36) ng / ml] were significantly lower than that of the control group [(6.37 ± 1.35) pg / ml, ] (P <0.05). Pearson correlation analysis showed that the levels of Treg cells in both groups were negatively correlated with the levels of Th17 cells and IL-17 (r = -0.32, -0.54, P <0.05, respectively) α levels were positively correlated (r = 0.47,0.61, P <0.05). Conclusions The level of Treg cells in patients with rheumatoid arthritis complicated with latent tuberculosis infection is decreased. The level of Treg cells is closely related to tuberculosis infection.
