目的探讨解冻移植(FET)周期中第3天(D3)胚胎解冻后当天移植和解冻后过夜培养后移植对临床结局的影响。方法回顾性分析本中心2013年5月-12月551例移植胚胎数≥2个D3胚胎FET周期临床结局,根据解冻后胚胎培养时间分为当天移植组(2h-3h)和过夜培养移植组(18h-20h)。再将两组按解冻前胚胎级别分为优胚组(≥2个II级胚胎)、混合胚组(1个II级+≥1个III级胚胎)和一般质量胚胎组(≥2个III级胚胎)。结果 FET周期中,对优胚、混合胚胎和一般质量胚胎,18h-20h组和2h-3h组年龄、平均移植胚胎数和种植率均没有显著差异(P>0.05)。对各级别胚胎,18h-20h组临床妊娠率较2h-3h组有了一定提高(分别为43.21%vs 37.27%;31.37%vs 28.13%;29.41%vs 21.52%),但差异不显著(P>0.05)。结论 D3胚胎解冻后过夜培养(18h-20h)不能显著提高D3胚胎FET周期临床妊娠结局。但是,提前解冻有利于实验室灵活安排工作。
Objective To investigate the effect of posttransplantation on the clinical outcome after thawing and thawing on day 3 of thawed embryo transfer (FET). Methods Retrospective analysis of the clinical outcomes of FET cycle of 551 embryos with more than or equal to 2 D3 embryos from May 2013 to December 2013 in our hospital was made according to the embryo culture time after thawing and the day of transplantation (2h-3h) 18h-20h). The pre-thawed embryos were divided into two groups according to their pre-thawed embryo status (≥2 grade II embryos), mixed embryos (grade II + ≥1 grade III embryos) and normal quality embryos (≥2 grade III Embryo). Results There was no significant difference in the age, mean number of embryos transferred and planting rate in excellent embryo, mixed embryo and general quality embryos among 18h-20h and 2h-3h groups in FET cycles. The clinical pregnancy rate of 18h-20h group was higher than that of 2h-3h group (43.21% vs 37.27%, 31.37% vs 28.13%, 29.41% vs 21.52%, respectively) for all levels of embryos, but the difference was not significant (P> 0.05). Conclusion The incubation of D3 embryos overnight (18h-20h) did not significantly improve the clinical pregnancy outcome of FET cycles of D3 embryos. However, thawing in advance facilitates flexible laboratory work.
