目的探讨彩色多普勒超声(CDFI)以及多普勒超声流速曲线对于良恶性腋下淋巴结进行鉴别的临床价值。方法选取2012年6月-2017年1月本院收治的29例乳癌患者的10枚阳性腋下淋巴结和19枚阴性腋下淋巴结作为观察组;选取同期乳腺良性病变患者的11枚腋下淋巴结和正常人群的18枚腋下淋巴结作为对照组,对两组腋下淋巴结的CDFI密度、CDFI分布以及淋巴结内的血流峰值速度(Vmax)和阻力指数(RI)进行对比分析。结果乳癌腋下淋巴结阴性组、乳腺良性病变组以及正常人群组的腋下淋巴结的CDFI密度等级比较差异不具有统计学意义(P>0.05),乳癌腋下淋巴结阳性组的CDFI密度等级与其他三组比较差异具有显著的统计学意义(P<0.05);通过对多普勒超声流速曲线进行分析,结果发现各组研究对象的血流峰值速度(Vmax)和阻力指数(RI)差异不明显,不具有统计学意义(P>0.05)。结论彩色多普勒超声对于良恶性腋下淋巴结的鉴别具有较高的临床应用价值,但是多普勒超声流速曲线对于良恶性腋下淋巴结的鉴别不具有实际意义。
Objective To investigate the clinical value of color Doppler ultrasound (CDFI) and Doppler flow velocity curves in differentiating benign and malignant axillary lymph nodes. Methods Ten positive axillary lymph nodes and 19 negative axillary lymph nodes from 29 patients with breast cancer admitted to our hospital from June 2012 to January 2017 were selected as the observation group. Eleven axillary lymph nodes and The 18 underarm lymph nodes in normal population served as the control group, and the CDFI density, CDFI distribution, peak blood flow velocity (Vmax) and resistance index (RI) in the axillary lymph nodes were compared between the two groups. Results There was no significant difference in CDFI density level between axillary lymph nodes in breast cancer undectum lymph node group, benign breast lesions group and normal human group (P> 0.05). The density of CDFI in axillary lymph node positive breast cancer group was higher than other groups The difference between the three groups was statistically significant (P <0.05). By analyzing the Doppler flow velocity curves, there was no significant difference in Vmax and RI between the three groups , Not statistically significant (P> 0.05). Conclusions Color Doppler ultrasound has a high clinical value in the differential diagnosis of benign and malignant axillary lymph nodes. However, the Doppler ultrasound velocity curve is not meaningful for differentiating benign and malignant axillary lymph nodes.
