目的:探讨妊娠期糖尿病(GDM)产妇的孕期管理及血糖控制水平与母婴预后的关系。方法:以2008~2010年在济南市妇幼保健院住院分娩的23 702例产妇为研究对象,对被确诊的692例GDM患者的临床资料进行分析,血糖控制满意的579例患者为Ⅰ组,血糖控制不满意的113例患者为Ⅱ组,对两组的孕妇并发症及新生儿结局进行比较。结果:GDM患病率为2.92%,并逐年递增。Ⅱ组子痫前期、巨大儿、早产、新生儿低血糖等发生率明显高于Ⅰ组,差异有统计学意义(P<0.05),两组剖宫产、新生儿窒息、新生儿呼吸窘迫综合征发生率比较差异无统计学意义(P>0.05)。结论:加强GDM管理、严格控制GDM患者的血糖水平能改善妊娠结局。
Objective: To investigate the relationship between pregnant women with gestational diabetes mellitus (GDM) during pregnancy management and blood glucose control and the prognosis of maternal and infant. Methods: A total of 23 702 mothers who were hospitalized and delivered in Jinan MCH hospital from 2008 to 2010 were analyzed. The clinical data of 692 confirmed GDM patients were analyzed. 579 patients who were satisfied with blood glucose control were Group Ⅰ, The 113 patients who were not satisfied with the control were group Ⅱ, and the complications and neonatal outcomes of the two groups were compared. Results: The prevalence of GDM was 2.92%, increasing year by year. The incidence of preeclampsia, macrosomia, premature birth and neonatal hypoglycemia in group Ⅱ was significantly higher than that in group Ⅰ (P <0.05). The incidence of cesarean section, neonatal asphyxia and neonatal respiratory distress syndrome There was no significant difference in the incidence of symptoms (P> 0.05). Conclusion: To strengthen GDM management and strictly control the blood sugar level of GDM patients can improve pregnancy outcomes.
