目的 探讨低密度脂蛋白胆固醇 (LDL C)对首次急性心肌梗死后左室收缩功能的预测价值。方法 对 40例首次急性心肌梗死行静脉溶栓治疗的患者 ,静脉采血化验心肌酶 (CPK MB) ,血脂及其他生化指标。Doppler超声测左室功能 (EF值 )。按低密度脂蛋白胆固醇 (LDL C)的水平分为正常LDL C组及高LDL C进行比较。结果 正常组左室射血分数 (LVEF)为 0 .6 4± 0 .0 6 5明显高于高LDL C组 0 .5 5± 0 .0 47(P <0 .0 1)。血清肌酸磷酸激酶同工酶峰值为 (143 .2 3± 81.48)U/L明显低于高LDL C组 (195 .82± 77.39)U/L(P <0 .0 5 )。总胆固醇 (TC)、甘油三酯 (TG)、LDL C以及CPK MBmax与LVEF间呈负相关 ,尤其是LDL C与LVEF间呈显著负相关。结论 低密度脂蛋白胆固醇对首次急性心肌梗死后左室收缩功能有较高的预测价值。
Objective To investigate the predictive value of low density lipoprotein cholesterol (LDL C) on left ventricular systolic function after first acute myocardial infarction. Methods Forty patients with acute myocardial infarction who underwent intravenous thrombolysis were divided into two groups: venous blood test cardiomyocyte enzyme (CPK MB), lipids and other biochemical parameters. Doppler ultrasound measured left ventricular function (EF value). According to the level of low density lipoprotein cholesterol (LDL C) is divided into normal LDL C group and high LDL C for comparison. Results The left ventricular ejection fraction (LVEF) of the normal group was 0.664 ± 0.605, which was significantly higher than that of the high LDL group, 0.55 ± 0.47 (P <0.01). Serum creatine phosphokinase isoenzyme peak (143.23 ± 81.48) U / L was significantly lower than the high LDL C group (195.82 ± 77.39) U / L (P <0.05). There was a negative correlation between total cholesterol (TC), triglyceride (TG), LDL C, and CPK MBmax and LVEF. In particular, there was a significant negative correlation between LDL C and LVEF. Conclusion Low-density lipoprotein cholesterol has a higher predictive value of left ventricular systolic function after first acute myocardial infarction.
