目的:探讨64层螺旋CT冠状动脉成像的临床应用价值。方法:100例临床诊断可疑冠心病患者进行64层螺旋CT冠状动脉成像,男性65例,女性35例,年龄35~78岁,平均年龄56.5岁。回顾性心电门控增强扫描,利用多层面重建(MPR)、最大密度投影(MIP)及容积再现技术(VRT)重组图像,判定冠状动脉病变情况,并与DSA对照。结果:100例患者中,左前降支、左旋支、右冠状动脉重组像评价图像质量,在心率<70次/min时,左前降支、左旋支、右冠状动脉显示较好且能满足影像学评价。MPR、MIP及VRT重组像能显示冠状动脉的所有1级、大部分2、3级以及部分4级分支。64层螺旋CT冠状动脉成像评价≥50%,冠状动脉狭窄的敏感性为94.0%,特异性为94.0%,符合率79.0%。结论:64层螺旋CT冠状动脉成像是一种无创、快速的成像方法,在多数情况下能较好地显示冠状动脉,可以做为冠状动脉病变的筛选方法。
Objective: To investigate the clinical value of 64-slice spiral CT coronary angiography. Methods: Sixty-four spiral CT coronary angiography was performed in 100 patients with suspected coronary heart disease. There were 65 males and 35 females, aged from 35 to 78 years with a mean age of 56.5 years. Retrospective ECG-enhanced scanning was performed to evaluate the coronary artery lesions by using multi-faceted reconstruction (MPR), maximum density projection (MIP) and volume rendering (VRT) images. Results: In 100 patients, left anterior descending coronary artery, left circumflex artery and right coronary artery were used to evaluate the image quality. When the heart rate was less than 70 beats / min, the left anterior descending coronary artery, the left circumflex artery and the right coronary artery showed better imaging quality Evaluation. MPR, MIP and VRT recombination images can show all levels 1, 2, 3, and 4 of the coronary arteries. The evaluation of 64-slice spiral CT coronary angiography was ≥50%. The sensitivity and specificity of coronary stenosis were 94.0%, 94.0% and 79.0% respectively. Conclusion: 64-slice spiral CT coronary angiography is a noninvasive and rapid imaging method that can display coronary arteries in most cases and can be used as a screening method for coronary artery disease.
