目的比较膈肌肌电均方根(RMSdi)与积分肌电值(i EMGdi)两种膈肌电信号处理方法对气道阻力增加状态下呼吸中枢评价的准确性。方法选择2015年1-6月广州12名健康志愿者,接受不同程度的吸气阻力,并测量膈肌肌电以及跨膈压,膈肌肌电分别采用均方根和积分值处理,两种算法得出的结果分别与吸气跨膈压作相关性分析,比较其相关系数。结果在低、中、高水平吸气阻力时,膈肌肌电均方根与吸气跨膈压之间的相关系数分别为0.198,0.141及0.569(P=0.054),均无相关性(均P>0.05);而3个阻力下的积分肌电值与吸气跨膈压之间的相关系数分别为0.896,0.885及0.876,均存在明显相关性(均P<0.01)。结论吸气阻力存在时,积分肌电值比膈肌肌电均方根更能准确地评价呼吸中枢的吸气努力程度。
OBJECTIVE: To compare the accuracy of the diaphragmatic electrical signal processing methods for the evaluation of the respiratory center under the condition of increased airway resistance by comparing the RMSdi and EMGdi values of diaphragm electromyography. Methods Twelve healthy volunteers in Guangzhou from January to June in 2015 were selected to receive different degrees of resistance to inhalation and to measure diaphragmatic myoelectricity and diaphragmatic pressure and diaphragmatic myoelectricity using root mean square and integral values respectively. The results were correlated with inspiratory diaphragmatic pressure, and the correlation coefficients were compared. Results At the low, medium and high levels of inspiratory resistance, the correlation coefficients between the mean square root of diaphragmatic muscle tone and inspiratory diaphragmatic pressure were 0.198, 0.111 and 0.569, respectively (P = 0.054) > 0.05). The correlation coefficients between the myoelectric values of the three resistances and the diastolic diastolic pressure were 0.896, 0.885 and 0.876, respectively (all P <0.01). Conclusion In the presence of inspiratory resistance, the value of integral myoelectricity is more accurate than the mean value of the myoelectric root mean square of diaphragmatic muscle in evaluating the inspiratory effort of the respiratory center.
