闭合性肘关节脱位是常见的,仅次于肩关节脱位,但伴有肱动脉横断却是罕见的。1900年以来在英文文献中只报道过14例。本文报道第15例,复习了文献并提出治疗的方法。病例为一63岁女性,摔倒后右手掌着地当即右肘部疼痛。一小时后急诊检查发现右肘关节不能活动,右手指麻木,右肘明显肿胀变形,无皮损,上臂中段可触及肱动脉搏动,Doppler血流仪未测出桡动脉和尺动脉搏动。X线片显示肘关节向后外脱位,无骨折。经闭合复位及以1%利多卡因3 ml浸润右肘前肱动脉
Closed elbow dislocation is common, after the shoulder joint dislocation, but with brachial artery transection is rare. Since 1900, only 14 cases have been reported in English literature. This article reports the 15th case, reviewed the literature and proposed treatment methods. The case was a 63-year-old woman who immediately fell right hand palm pain right elbow. One hour later, the emergency examination revealed that the right elbow joint could not move, the right finger was numb, and the right elbow was obviously swollen and deformed with no lesions. The brachial artery pulse was touched in the middle of the upper arm. The radial artery and ulnar artery pulse were not detected by Doppler flowmeter.
